Position Summary

:

The Wrap Services and Housing Facilitator plays a key role in delivering client-centered, trauma-informed support to women and gender-diverse individuals experiencing criminalization, homelessness, or housing instability. This position supports clients in navigating housing systems, accessing wraparound services, and maintaining housing through intensive case management and community partnerships. This position requires strong communication skills, conflict resolution, and trauma informed approaches to case management.

Key Responsibilities

:

Client Support & Housing Stabilization

Provide direct, individualized support to clients experiencing housing instability, including those exiting incarceration, fleeing violence, or facing homelessness. Assist clients in navigating housing applications, viewings, lease agreements, and move-ins. Collaborate with clients to create and implement housing support plans and goals. Provide support in maintaining housing (e.g., budgeting, cleaning, landlord relations, conflict resolution). Assist with eviction prevention through advocacy, system navigation, and referrals. Assist clients in accessing emergency shelters.
Case Management & Advocacy

Conduct intakes, assessments, and regular check-ins with clients to monitor progress and well-being. Maintain accurate and confidential case notes and documentation in accordance with agency policies and funder requirements. Connect clients with appropriate services, such as income support, mental health, addictions services, food security programs, and employment readiness. Advocate on behalf of clients with housing providers, legal systems, and community services. Provide individualized case management to clients navigating housing instability, mental health, addiction, and justice system involvement.
Facilitation & Site Support

Support the safety and operations of Elizabeth Fry housing sites, including the Charles Street Women's Centre and transitional housing units. Monitor adherence to housing agreements and support conflict resolution as needed. Participate in staff meetings, supervision, and case conferencing. Facilitate wraparound support services, including connections to healthcare, income support, legal advocacy, addiction and mental health services, and employment programs. Conduct risk assessments, safety planning, and crisis intervention as needed. Participate in team meetings, case conferences, training, and organizational events. Promote a safe, inclusive, and nonjudgmental environment for all clients.
Community Engagement & Systems Navigation

Build relationships with landlords, housing partners, shelters, and social service agencies to enhance housing options and wraparound supports. Stay current with local housing policies, community resources, and system barriers affecting clients. Maintain regular communication and follow-ups with clients to assess progress, address emerging needs, and promote long-term stability. Grand Valley client assessment, interview, relationship-building with federally sentence women requiring wrap-around services

Qualifications

:

Post-secondary education in Social Work, Social Services, Community Justice, Human Services, Psychology or a related field (equivalent experience considered) Minimum 2 years of experience working in supportive housing, homelessness services, or a related social service environment Strong knowledge of housing systems, income supports, and housing retention strategies Demonstrated understanding of trauma-informed, anti-oppressive, and harm reduction approaches Excellent interpersonal, communication, and problem-solving skills Ability to work independently and collaboratively within a team Valid driver's license and access to reliable transportation is an asset Clear criminal record and vulnerable sector check required upon offer of employment

Working Conditions

:

Combination of on-site, community-based, and office work May require occasional evening or weekend work based on client needs or housing emergencies Physical ability to assist with move-ins or attend housing inspections may be required
