Position Summary:
The purpose of the Wait List Coordinator position is to effectively engage, screen, assess, refer, provide brief interventions and coordinate care for people aged 65 + years that contact or are referred to the District Mental Health Services for Older Adult Program.
As one of the first contact points for people accessing CMHAFF, the Wait List Coordinator will need to build a positive rapport with a wide range of people.
The Wait List Coordinator will need to be skilled in assessing an older adult person's needs, including complex risk assessments and the ability to action a plan to mitigate any identified risk. The Wait List Coordinator will be supported by regular supervision.
The Wait List Coordinator works as part of a team, including general practitioners, allied health clinicians, community engagement workers, and support staff. They will also work closely with local community and health services to provide holistic and integrated care. They will provide supports across the Rainy River and Kenora Districts working in conjunction with the DMHSOAP team. This is a hybrid position allowing the staff to work on site and remotely. There would be minimal travel required.
Essential Duties:
Conduct risk assessments including assessment of suicide risk and violence risk, developing action plans to mitigate any risks, and providing follow up support and referral for treatment. High risk clients must be promptly reported to the supervisor for appropriate action.
Monitor clients for critical changes and initiate appropriate emergency procedures.
Refer people to internal and external services as appropriate and provide follow up support.
Provide brief interventions as needed to clients on the waitlist.
Works in a manner that preserves, maintains and respects privacy and confidentiality of client and staff information.
Manages wait list that includes on going assessment, treatment planning and therapeutic interventions for older adults with a serious mental illness and their caregivers.
Provides consultation, education and liaises with other staff, community organizations and professionals in a monitoring way to older adults with a serious mental illness and their caregivers.
Provides information, direction, advocacy and referral for older adults and their caregivers that require access to other local, district and regional resources.
Maintain up-to-date client records and comply with data collection standards.
Build and maintain effective relationships with key stakeholders involved in the provision of health, mental health and psychosocial services to people.
Works in a manner that considers and promotes client safety.
Works in a manner that compiles with the Branch's policies and procedures and takes responsibility for awareness and understanding of said policies and procedures.
Participate in relevant training and development activities as an effective team member.
Other duties consistent with the position where required and/or requested by leads.
Contributes to the Branch's activities to collect, analyze and report on data, and participate in research.
Research and develop a list of resources across the Kenora and Rainy River Districts.
Qualifications:
Proof of current full registration with one of the colleges that is permitted to perform the controlled act of psychotherapy.
The ability and experience to undertake intake and screening activities, including comprehensive bio-psycho-social assessment to determine client needs, and to assist clients to access appropriate services.
Experience working with individuals who have a serious mental illness or dementia.
The ability and experience to conduct risk assessments, including suicide and violence risks, and to develop action plans that mitigate these risks.
The ability and experience to be able to deliver brief therapeutic interventions, counselling services and treatment planning.
Ability to liaise with clients, family members, professionals and agencies of varying ideologies.
Knowledge and understanding of geriatric mental health, including related evidence-based interventions and clinical practice.
Exceptional interpersonal skills with the ability to work with a diverse range of people, in particular the ability to engage with people and advocate on their behalf.
Highly developed verbal and written communication skills.
Excellent organizational and time management skills, including the ability to prioritize and manage multiple and competing work tasks and deliver to agreed deadlines.
Excellent computer skills
Ability to work both independently and collaboratively as a productive team member.
Valid driver's license and access to a vehicle, agency prescribed auto insurance, valid first aid/CPR certificate.
Current vulnerable sector criminal record check.
