Job Description


JOB PURPOSE

As a member of an interdisciplinary team, provides quality patient care within the scope of professional nursing practice to patients. Performs other duties as assigned. Although primarily assigned to Medical floor, may be reassigned to other units as required.

QUALIFICATIONS

  • Currently registered with the Nurses Association of New Brunswick.
  • Knowledge, experience and interest in working with Geriatric and cognitively impaired patients
  • Knowledge and experience in Post-op Surgical Care would be an as asset
  • Demonstrates effective clinical competency and critical thinking.
  • Demonstrates and understands the nursing process
  • CPR certification.
  • Accucheck Machine certification
  • Proven ability to work independently and as a member of an interdisciplinary team.
  • Ability to perform Charge Nurse responsibilities.
  • High regard for confidentiality and advocacy for patients.
  • Ability to communicate effectively and pleasantly with patients, families, staff, physicians and the general public.
  • Demonstrates initiative, good judgment and decision-making abilities that contribute to positive health outcomes.
  • Ability to attend work on a regular basis.
  • Ability to perform the duties of the position.
  • Good work record.
KEY RESPONSIBILITES
  • Possesses the specialized nursing knowledge and clinical skills necessary to provide care and to supervise and instruct subordinate staff in the care of geriatric patients.
  • Assess the health care needs of patients including patients assigned to non-professional staff
  • Responsible for ensuring appropriate patient assignments based on patient conditions and/or staffing changes.
  • Maintains the standards of clinical care on the unit as per the inpatient nursing standards.
  • Acts as resource for wound care, infection prevention and control, falls prevention, and Dementia care.
  • Leads the development of and continuously evaluates effectiveness of individual nursing care plans.
  • Strong clinical assessment skills for geriatric patients.
  • Identifies patient needs and problems utilizing an assessment tool
  • Formulates objectives specific to patient needs
  • Provides nursing care required to achieve the patients\xe2\x80\x99 desired outcomes
  • Evaluates outcomes of nursing actions for further assessment and planning
  • Identifies needs effecting discharge planning and potential services required at home
  • Utilizes effective communication skills to achieve a helping relationship
  • Acts as a patient advocate, promoting an environment conducive to open communication and professionalism.
  • Clinical practitioner who leads nursing practice.
  • Collaborates with interdisciplinary team members including liaison with physicians.
  • Management of end-of-life care.
  • Provides health teaching, counseling, health promotion to individuals and groups
  • Supports new employees during orientation
  • Geriatric education as required.
