About Fraser Street Medical (FSM)
Fraser Street Medical (FSM) is a vibrant, UBC-affiliated interdisciplinary family medicine
clinic located in Vancouver. Our clinic is dedicated to collaborative, patient-centered care
through an innovative team-based model that includes physicians, nurse practitioners,
chronic disease coordinators, medical office assistants, and a growing number of allied

About the Role - Registered Nurse (RN)
We are hiring two full-time RNs (2.0 FTE total) under BC's Nurse in Practice Program, in
partnership with the Ministry of Health and FPSC. Nurses will support chronic disease
management, panel optimization, urgent care access, and team-based integration. Each RN
will be assigned to a clinician pod and will function at full scope to proactively manage
patient care and offset provider burden.
Nurses will have access to mentorship and coaching through the NNPBC Nursing Initiatives
for Primary Care (NIPC) program, including regional consultants, an online network, and

FSM is committed to onboarding and supporting nurses in skill development. Candidates
are not expected to fulfill every competency on day one and will be supported to grow into
the role with coaching, team mentorship, and time.
Core Responsibilities
1) Direct Clinical Care (?70% FTE)
- Provide ?1,628 direct care hours/year and target ~4,400 RN-led visits per FTE
- Triage same-day clinical inquiries via phone and email
- Manage minor concerns (e.g., UTI protocol, BP support), provide advice, and escalate as
appropriate
- Deliver independent, guideline-informed care for diabetes, hypertension, COPD, CHF, and
preventive services
- Facilitate group medical visits and self-management counselling
2) Chronic Disease Panel Recalls (30% FTE)
- Use EMR dashboards to recall patients overdue for care
- Apply evidence-based protocols for high-risk chronic diseases
- Collaborate with allied health professionals for referrals and education
Panel Management, QI, and Team Coordination
- Attend monthly CDM huddles with clinician pod
- Participate in PSP QI initiatives focused on attachment, CDM improvement, continuity, and
access
- Support documentation standards and patient education development
Collaborative Team Model
- FSM CDM Nurses: deliver autonomous RN care, reduce backlog, improve third-next
availability
- CDM Coordinator: manages recalls, vaccinations, pharmacist referrals, and QI data
- HVA: manages incoming documents, labels labs, triages tasks, supports administrative
follow-ups

Required Qualifications
- RN in good standing with the BC College of Nurses and Midwives (BCCNM)
- Bachelor's degree in Nursing (B.N. or B.Sc.N) preferred
- 1-3+ years in primary care or community health preferred
- Skilled in chronic disease care and independent visits
- Proficient with EMRs (Med Access preferred)
- Basic Life Support (CPR-C)
Skills and Competencies
- Strong decision-making and prioritization skills
- Ability to apply trauma-informed care and cultural safety in all patient interactions
- Clinical competence in primary care nursing and chronic disease management
- Conflict resolution and crisis intervention capability
- Ability to work independently and collaboratively
- EMR proficiency and ability to work with data for care gap identification
- Demonstrates sound judgment, confidentiality, and the ability to organize and prioritize
service delivery
Assets
- Experience with chronic disease group medical visits
- Knowledge of panel management, QI cycles, and EMR dashboards
- Familiarity with Ministry-funded longitudinal care models (e.g., LFP billing, third-next
targets)
- Multilingual (e.g., Punjabi, Tagalog, Cantonese)
Work Environment
- In-person care with virtual and phone-based follow-up. No home visits expected
- Flexible, collaborative approach encouraged to adapt to clinic and patient needs
- Support provided by FSM's CDM Coordinator, HVA, and administrative leadership

- Observes and promotes universal precautions to contribute to a safe and healthy working
environment
- Participates in FSM's emergency preparedness, fire prevention, and infection control
protocols
Communication & Documentation
- Maintain accurate and timely documentation in accordance with BCCNM documentation
standards and clinic policies
- Reconfirm delegated medical directives are followed when appropriate and ensure clarity
on scope-based activities
- Ensure secure handling of all patient data and uphold clinic confidentiality policies
- Provide team members with timely updates and patient-specific instructions as needed
Education, Equity, and Professional Development
- Model cultural humility and demonstrate commitment to anti-racism, inclusion, and
trauma-informed care
- Participate in ongoing training, team development, and quality improvement initiatives
- Support and educate patients with diverse needs and backgrounds
- Maintain a commitment to professional development and adherence to regulatory
standards
Professional Role and Development
- Optional participation in medical education and clinic-based CME events (e.g., lunch-and-
learns, evening sessions supporting chronic disease education)
- Active contribution to FSM's quality improvement initiatives and pathway development
- May contribute to clinic-based research, policy advocacy, or student mentorship projects

- Medication reconciliation

- Wart treatment and ear syringing
- Supporting advance care planning and end-of-life discussions

Note: All interventions must be performed within RN scope as outlined by BCCNM and may
be subject to medical directives or clinic-specific protocols.
Salary Range
As required by the BC Pay Transparency Act, salary for this position is between $40.21 -
$54.28 per hour, based on experience and qualifications
