WellFort is seeking a Support Care Navigator to join our team!
WellFort Community Health Services is an innovative community health centre that is embarking on a transformational change to support the population through integrated care services across interprofessional primary care models. We are hiring multiple roles that focus on a number of patient populations across all age spans including within a new neighbourhood primary care network of 7 primary care practices, homeless shelters and community locations serving people experiencing homelessness and our hub locations in Malton and Bramalea. Multiple roles (contract and permanent) and multiple FTE's are being recruited for. Work in all roles could be across any program among multiple sites within the Central West Ontario Health Team geography. If interested, please identify your FTE availability within your application. The Support and Care Navigator assists the client along their journey through WellFort's programs and services. The SCN may be assigned to focus on one or more stages of the client's journey, but generally they will have the capacity to support clients through intake, assessment and service delivery and provide instrumental and community support as needed, working within WellFort's service delivery frameworks.
The SCN works as part of an inter-disciplinary team functioning both independently and with other staff in client support and education, as well as participating in WellFort outreach to priority populations
RESPONSIBILITIES
Care Navigator Support in the Centre
As part of the circle of care supports clients with their care plans including Health Links
Collaborates and delivers Shared medical appointments and other wellbeing initiatives
Collaborates and delivers health promotion, education, prevention, and CDSM workshops
Conducts intake interviews to ensure that clients meet WellFort's mandate
Registers clients and collects socio economic demographic and other relevant information
Conducts client assessments such as PHQ9's etc.
Leads Client Advisory Committees, client engagement and feedback mechanisms and client experience surveys
Provides a comprehensive introduction to all eligible services
Connects clients to appropriate community and social supports
Provides a range of instrumental supports (forms, interpretation, and internal referrals)
Initiates the discharge process and plans for clients as appropriate
Conducts annual updates of socio-economic demographics
Maintains records, forms and electronic encounter data as required
Ensures that the welcome area is welcoming and user friendly
Provides back-up support for the front desk as required
Client Adviser, Support and Education in the community
Provides clients with support services, e.g. external referrals and information regarding community services, shelters, government services, partner agencies; assists clients with access to items such as clothing, food and needle exchange
Provides occasional accompaniment to services e.g. emergency room, specialist appointments, shelters or other agencies
Provides information on internal and external resources as required
Health Promotion
Design and evaluate health promotion programs and activities to support clients and community members to make healthy lifestyle choices, while addressing issues that limit those choices and to enhance ability of vulnerable residents to function independently, improve or protect the environment and support the development of healthy public policy and practice.
Supports the Health Promoter to identify and address local health and social issues that impact the health of Bramalea or Malton residents, through research
Engage community groups and networks in support of community initiatives and represent WellFort at meetings with those groups.
Represent WellFort at collaborative partnership meetings such as community health week, Peel Pride, cancer screening initiatives, etc.
Identify assets and resources that can be used to build a healthier community
Coordinate and align health promotion initiatives with delivery of community health education programs organization wide, including health education in primary and allied care and diabetes education.
Design instruments for disseminating knowledge to and soliciting knowledge from clients, such as health education materials in the welcome rooms, client surveys and focus groups.
Multimedia
Care Navigator Support creates content for WellFort's Website, FrontFace and Social Media accounts, related to Health Education and Health Promotion, programs and activities. Multimedia resources include and are not limited to promotional material, videos and infographics, etc.
Administration
Assists and supports the completion of funding proposals, grant applications, evaluations and other periodic written reports and presentations, as required
Supervises and provides support for students from various disciplines with a particular focus on the determinants of health, primary health care and role of community development professionals within the CHC.
Develops and implements WellFort's health equity plan
Develop and implement an annual work plan and work plan tracker in collaboration with direct supervisor
Participates in research when opportunities arise.
Provide care delivery at any of the office sites as program needs dictate.
Consistent access to a vehicle for responsive care delivery at any care delivery location
Other duties and responsibilities as may be assigned from time to time.
QUALIFICATION, EXPERIENCE & SKILLS
An undergraduate degree in a relevant field.
Minimum of two years' experience working for a community-based health/services and/or values-based organization.
Knowledge of health and social service providers, their policies, and support networks in Brampton and Mississauga. Familiar with community resources and government services.
Ability to establish good working relationships with partner agencies which provide the continuum of health and social sector services to clients.
Excellent written communication and oral skills that meet the needs of the individual and/or community at the appropriate literacy level.
Experience working with confidential and time sensitive documents and information.
Excellent interpersonal skills and the ability to multi-task in a fast-paced environment.
Experience working in a value based non-profit or social services organization.
Proven ability to work both independently and collaboratively in an interdisciplinary team where skills and responsibilities are recognized and respected
Demonstrated proficiency in the use of computers and various computer software applications, such as Electronic Medical Record (EMR).
Ability to speak languages relevant to the residents of the organization's community is an asset.
Some travel may be required so candidate must possess a valid driver's license and have access to a reliable vehicle with current insurance.
