Job Summary
As the Supervisor, Personal Support Services (PSS), you are responsible for daily delivery of personal support services provided by PSWs. You will complete personal support care plans with new clients, update care plans as required, engage with service funders (primarily Ontario Health at Home Care Coordinators), participate in client case conferences, and support the resolution of identified service concerns. In collaboration with the Professional Practice Team, you will support the orientation of new staff, provide ongoing staff training, follow up with any identified job performance issues and complete regular performance reviews. In doing so, you will ensure high quality personal support services are delivered to Carefor clients. Further, you will use evidence-based, best practice guidelines supported with informational data to ensure successful team performance. This position description outlines the key responsibilities and the nature of work. It is not a comprehensive inventory of all duties and tasks of the position.
Primary Responsibilities Include
Oversees and assists the Manager with the clinical operation of the PSS program
Acts as a resource, support and mentor to the PSS staff by leading and coaching staff to attain individual and operational performance objectives
Contributes to human resources management by supporting labour and employee relations which includes performance development, management, and recognition.
Ensures Carefor policies & procedures affecting the delivery of Personal Support Services are implemented by PSW staff
Completes annual performance reviews on assigned Personal Support Services staff
Identifies and discusses with the Manager employee issues requiring disciplinary action
Assists in determining the required staffing complement based on identified needs
Assists Managers in the employee recruitment process by anticipating and monitoring staffing needs and initiating appropriate action to obtain/maintain an adequate compliment of staff
Participates in a regular after hours On Call rotation to support urgent operational needs outside of regular business hours.
Operational And Organizational Responsibilities
Understands, promotes, and communicates the value, purpose, and relationship of the Personal Support Services program to Carefor as part of its strategic plan
Promotes continuous quality improvement by contributing to the implementation and evaluation of strategies designed to increase the provision of quality care
Works in collaboration with client/family to develop the Client Care Plan and complete safety assessments, updates them as required, and communicates changes with the funder and PSS care team.
Ensures that the quality of client care delivery is consistent with the funded contractual terms of care & provides regular updates to the funder regarding the care provided
Supports PSW scheduling by providing recommendations regarding client care needs
Investigates, evaluates and supports the resolution of identified client and staff concerns & complaints
Health And Safety Responsibilities
Operates within and monitors individual, staff, and program compliance to Occupational Health and Safety Legislation, standards, and Carefor Policies & Procedures
Participates in health and safety training including WHMIS and applies this knowledge
Works in collaboration with Management and Human Resources to complete return to work plans for Personal Support Workers taking part in modified work
Problem Solving
Problem-solving skills are required to make recommendation to offer solutions to issues and challenges relating to people issues, and Carefor operating policies and practices.
Decision Making/Independence Of Action
Makes recommendations for continuous improvement relating to Carefor's overall objectives, policy, and procedures. Decisions may require review or escalation when costs, potential liability, reputational risk and/or an impact on objectives or Personal Support Services policies are involved.

Physical effort required is low intensity, <2 hours per day
Mental Effort
Mental effort required is moderate intensity in an office environment, 2-4 hours per day, or high intensity for less than 2 hours per day.

Qualifications
A Registered Practical Nurse (RPN), licensed to practice in Ontario, in good standing, is required
A minimum 1-year leadership is preferred
A minimum of 2 years community experience in Nursing or Personal Support Services
Demonstrated leadership abilities with highly developed organizational skills
Demonstrated understanding of CNO Standards of Practice
Excellent interpersonal and verbal/written communication skills
Excellent computer skills/knowledge and familiarity with scheduling systems
Ability to work collaboratively with internal and external stakeholders
A vehicle and a valid Ontario Driver's license are required
Ability to manage coinciding demands
Demonstrated flexibility and adaptability
Qualifications Considered an Asset
Experience working in a health care environment and/or working in a non-profit organization
Ability to converse in both official languages (English/French)
Experience providing Palliative Care and/or Dementia Care
Palliative certification (LEAP)
Gentle Persuasive Approach certification (GPA)
Previous experience working with unregulated care providers

The hiring range for this position is $36.67 - $43.96/per hour which reflects the expected starting salary based on qualifications and experience.
