OVERVIEW:

The Cowichan Tribes Ts'ewulhtun Health Center Sul'hween (Elders) Program is an interdisciplinary team consisting of Registered Nurses, Personal Care Worker (PCW) Team Lead, PCW's, Sul'hween Advocate, Administrative Coordinator, Program Drivers, Meals on Wheels Driver and Kitchen Staff.

PURPOSE:

Reporting to the Sul'hween Program and Nursing Manager, the Sul'hween Nurse Lead is responsible for providing day-to-day professional guidance, feedback, support and leadership to the Nursing team and PCW Team Lead and provide registered Nursing Services to Cowichan Sul'hween. The successful candidate will work collaboratively with Ts'ewulhtun Health Centre staff, Cowichan Tribes Sul'hween and other health service providers to implement best practices and quality standards to ensure effective delivery of Nursing services to meet Sul'hween health and wellness needs. This is a full-time, 35-hour per week, permanent position that works primarily in office with moderate to local travel required.

COMPENSATION AND BENEFITS:

Cowichan Tribes offers an excellent benefit and pension plan available after successful completion of the probationary period. The position includes paid training, paid vacation and paid sick leave. Depending on experience, the position starts at $53.29 - $55.85 per hour ($96,990.63 - $101,655.30 annually).

RESPONSIBILITIES:



Provide day-to-day professional guidance, feedback, support and leadership to the Nursing team and PCW Team Lead:



Ensure compliance of staff with all internal and external policies and procedures

Consult and debrief with Nursing Team and PCW Team Lead regarding Nursing Services and program activities and provides ongoing coaching and mentoring

Support team members and report to the Sul'hween Program and Nursing Manager on Nursing services, care plans related to Home and Community Care services, advocacy, referrals, special needs requests and other health related services

Monitor the workflow of assigned staffand determine appropriate assignment of staff resources

Ensure and maintain complete and accurate health records and case management files amongst the Nursing team

Participates and provides a leadership role in quality improvement and risk management activities by evaluating nursing practice, generating recommendations for alternative approaches in service delivery as per the Ts'ewulhtun Health Centre's commitment to the provision of safe family-centered quality care

Assist and participate with team members in development, implementation and evaluation of individual program activities

Advocate internally and externally for quality services and culturally safe interactions for Sul'hween; between First Nations and the medical community

Provide registered Nursing Services to Cowichan Sul'hween:



Provide chronic care management services to clients and administer flu vaccinations when required

Conduct home visits with clients

Assess client's current health conditions

Educate clients on their prescribed medication uses and dosages

Refer clients to internal and external resources when appropriate

Collect required client documentation for First Nations Health Authority's (FNHA) or Island Health's Occupational Therapist

Assess the safety of client homes and communicate risks to the appropriate department

Develop and update individual client care plans every six months, or as health status changes

Participate in team meetings and a variety of internal and external committees and working groups as assigned

Other related duties as required

EDUCATION AND EXPERIENCE:

Bachelor of Science in Nursing is required

Member of the Nurses and Nurse Practitioners of British Columbia is required

5 years' community nursing experience

2 years' Supervisory experience

A combination of relevant education and experience may be considered

Experience working in a First Nations community is preferred

Experience and/or willingness to learn about culture, customs, and traditions of Cowichan people

Certification in First Aid / CPR Level 1

Satisfactory Criminal Record Check with vulnerable sector check

Valid Class 5 BC Drivers' License and reliable vehicle

KNOWLEDGE, SKILLS, AND ABILITIES:



Knowledge of Home and Community Care and support services related legislation

Knowledge of complex chronic health conditions and the impact socioeconomic factors have on health

Knowledge of internal and external community resources

Knowledge of Cowichan culture, customs and traditions or willingness to learn

Ability to adapt to changing work environments, work priorities and the needs of a team

Well-developed written, oral, interpersonal and organizational skills

Excellent time management skills with the ability to multitask

Demonstrated skills in developing and maintaining positive relationships in the workplace

Demonstrated abilities leading effective teams
