FONCTIONS ET RESPONSABILIT\xc3\x89S PRINCIPALES:

Effectuer les activit\xc3\xa9s d\'inscription des patients, conform\xc3\xa9ment aux protocoles du programme et aux proc\xc3\xa9dures d\xc3\xa9partementales applicables

Proc\xc3\xa9der aux activit\xc3\xa9s de saisie des donn\xc3\xa9es d\'informations pertinentes re\xc3\xa7ues \xc3\xa0 diff\xc3\xa9rents points de contact pendant la prestation des services du programme

Fournir des r\xc3\xa9ponses aux appels de renseignements g\xc3\xa9n\xc3\xa9raux soumis par t\xc3\xa9l\xc3\xa9phone ou sur des plateformes Web sur les services du programme, l\'\xc3\xa9tat de la maladie ou les questions li\xc3\xa9es aux produits, en fonction des informations fournies dans le cadre du programme

Fournir un soutien m\xc3\xa9dical aux patients et aux prestataires de soins de sant\xc3\xa9, y compris, mais sans s\'y limiter, une \xc3\xa9ducation sur des maladies et th\xc3\xa9rapies sp\xc3\xa9cifiques et sur la gestion des maladies;

Pr\xc3\xa9parer et assister \xc3\xa0 des sessions de formation approfondies et sp\xc3\xa9cialis\xc3\xa9es pour des th\xc3\xa9rapies sp\xc3\xa9cifiques fournies par l\'\xc3\xa9quipe pharmaceutique

Support t\xc3\xa9l\xc3\xa9phonique (y compris les appels de maintenance et le soutien de l\'observance \xc3\xa0 la fois entrant et sortant selon les besoins);

Administrer et respecter les protocoles de prise en charge des cas, y compris la surveillance clinique, la collecte de donn\xc3\xa9es, le suivi continu des patients, l\'observance, la surveillance de l\'interruption / arr\xc3\xaat du traitement, la notification des \xc3\xa9v\xc3\xa9nements ind\xc3\xa9sirables et la liaison avec les fournisseurs de soins de sant\xc3\xa9 primaires;

Travailler directement avec le sp\xc3\xa9cialiste du remboursement et le m\xc3\xa9decin pour obtenir le financement des th\xc3\xa9rapies de leurs patients; aider \xc3\xa0 la recherche de financement alternatif au besoin;

Assister \xc3\xa0 des r\xc3\xa9unions de patients, \xc3\xa0 des conf\xc3\xa9rences au besoin pour informer les patients, les prestataires de soins sur la maladie / l\'information sur les produits et les services de gestion de cas;

Doit \xc3\xaatre en mesure d\'identifier et de signaler les EI \xc3\xa0 Sant\xc3\xa9 Canada, \xc3\xa0 Med Info et \xc3\xa0 entrer dans le syst\xc3\xa8me source et les syst\xc3\xa8mes tiers.

Maintient la confidentialit\xc3\xa9 des informations du client et de l\'entreprise et n\'en discute qu\'avec le personnel appropri\xc3\xa9

Faire la liaison avec les cabinets de m\xc3\xa9decins, les pharmacies, les fournisseurs de services et d\'autres parties pour fournir un service complet et coordonn\xc3\xa9 aux patients et aux m\xc3\xa9decins et r\xc3\xa9soudre les probl\xc3\xa8mes \xc3\xa0 mesure qu\'ils surviennent;

Specific Responsibilities

  • Perform Health Care Practitioner and patient registration activities, according to program protocols and applicable departmental procedures
  • Process data entry activities of pertinent information received at different points of contact during the delivery of program services
  • Provide answers to general inquiry calls submitted via telephone or web-based platforms on program services, disease state or product related questions, based on information provided as part of the program materials
  • Provide medical support to patients and health care providers, including but not limited to education on specific diseases and therapies and disease management;
  • Prepare for and attend extensive, specialized training sessions for specific therapies provided by the pharmaceutical team
  • Telephone support (including maintenance calls and adherence support both inbound and outbound as needed);
  • Administer and adhere to case-management protocols, including clinical monitoring, data collection, on-going patient follow-up, compliance, therapy disruption/discontinuation monitoring, adverse event reporting and liaison with primary healthcare providers;
  • Work directly with the reimbursement specialist and the physician to secure funding for their patient\'s therapies; assist with alternative funding searches when needed;
  • Attend patient meetings, conferences as needed to educate patients, providers regarding disease/product information and case management services;
  • Must be able to identify and report AE\'s to health Canada, Med Info and enter into source system and third-party systems.
  • Maintains confidentiality of client and corporate information and discusses same only with appropriate personnel
  • Liaise with physicians\' offices, pharmacies, services providers and other parties to provide comprehensive and coordinated service to patients and physicians and resolve issues as they arise;
Competencies
  • Registered Nurse, must be a member in good standing
  • 5-7 years of related experience
  • Knowledge of Microsoft Word, Excel, PowerPoint and other Office Programs
  • Strong computer and software skills: Excel, Word, Outlook, Adobe
  • Excellent written and verbal communication skills in English and French is preferred
  • Effective interpersonal and leadership skills

In addition to base pay, other compensation, such as an annual bonus or long-term incentive opportunities may be offered.

Our Base Pay Range for this position

$45,400 - $75,600
