As a member of the multidisciplinary healthcare team, the SW will apply their clinical knowledge and skills to provide comprehensive care, and follow-up to children ages 0-18.
Qualifications
Qualifications - External
1. Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker.
2. Baccalaureate degree in Social Work from an accredited School of Social Work, Master in Social Work preferred
3. 3-5 years clinical experience (within the last 6 years) with children and families dealing with acute and chronic medical illness, death and dying, bereavement issues and chronic progressive illness
4. Excellent knowledge of community support and placement services
5. Knowledge of Healthcare Consent Act, Substitute Decisions Act and Child and Family Services Act.
6. Proven ability to work as a member of interdisciplinary healthcare teams.
7. Demonstrated skills in the performance of a therapeutic role including strong assessment, counselling, case management and advocacy skills
8. Proven ability to work in a fast-paced, complex environment and to organize, prioritize and problem solve effectively
9. Demonstrated commitment to ongoing learning
10. Strong interpersonal, communication, teamwork, consultation and teaching skills
