Reporting to the Manager, Allied Health & Academic Liaison, the Social Service Worker & Admissions Coordinator is responsible for coordinating key aspects of the admission of new residents. The Coordinator will provide assessment, intervention, and evaluation services using social service work knowledge, skills and strategies to assist residents and their families in achieving optimal social functioning with the Home, including the provision of psychosocial and counselling supports as identified and needed. Working in collaboration with the Manager, Allied Health & Academic Liaison, nursing leadership team and members of the interprofessional team, the Social Service Worker & Admissions Coordinator will provide the services within the scope of practice and in alignment to the Vision, Mission, Values and Ethics of Providence Care.
Skills and Qualification Required:

  • Social Service Worker Diploma
  • Current registration with Ontario College of Social Workers and Social Service Workers
  • Experience working with seniors, families and vulnerable populations
  • Experience working in a Long-Term Care Home setting, preferred
  • Understanding of medical terminology, preferred
Position Duties & Responsibilities:
  • Conducts facility tours to prospective applicants/families
  • Provides information to potential applicants/families regarding the placement process
  • Acts as the Home's Lead liaison with community agencies and partners regarding the placement and ongoing support of prospective residents
  • Facilitates and completes the admission process for all permanent and short-term admissions in collaboration with the Director of Care, Assistant Directors of Care and the Charge Nurses
  • Facilitates internal resident transfers in collaboration with the resident/substitute decision maker, facility staff and other residents
  • Participates as a member of the interprofessional care team in developing and implementing resident care plans, resident mediation, and investigation in a person centric approach
  • Provides individual and/or group support for residents and families members and shares findings with care teams concerning the unique transition support required by the resident to foster adjustment to the new environment
