The Primary Health Care program is currently looking for a Social Navigator to work as a member within a community-based, primary health care interdisciplinary team. The mission of the project is to provide patient-centered, evidence-based, comprehensive, sustainable healthcare with excellence in quality, inclusion, service, and access. The principles of this project align with the patient health home model of care, using a collaborative health care team approach, from various health disciplines in the provision of primary health care services to patients at multiple sites across the metro region.As part of an interdisciplinary team, the Social Navigator will complete assessments and interventions, while supporting and advocating for patients within their care. The Social Navigator will assist patients to navigate through the health care system and ensure they receive appropriate and timely access to services across the continuum of care. The Social Navigator will also provide patient and family education; conduct risk assessments; assist clients in accessing required community services; help clients recognize, develop and utilize support networks; and initiate appropriate community referrals. The Social Navigator must possess a strong knowledge of the social determinants of health, health promotion, and chronic disease management. The Social Navigator will also be responsible for developing, implementing, evaluating and continuously improving the role in this setting.The Social Navigator will facilitate and participate in team consultation, huddles and case conferencing. They will also serve on committees and be involved in program planning and development, as required.The Social Navigator will be responsible for providing field instruction to social work students and offer other clinical education experience to students within Eastern Health. The Social Navigator reports to the Regional Primary Health Care Program Manager responsible for the service and to the Professional Practice Consultant for Social Work for integrity and professional practice issues. They will also be accountable to the external partners that they are working with (e.g., fee-for-service clinics).Flexible hours are required (Combination of days, evenings, and weekends required).Job Demands

Regularly sits to consult with clients, conduct therapeutic sessions, and to work on the computer. Occasionally works in awkward or cramped body positions; pushes, pulls, and sets up furniture for group work.
Drives to conduct home visits with clients.
Multi-site work environment.
The use of a personal vehicle is a condition of employment.Job Qualifications
Graduation with an accredited Bachelor of Social Work Degree is required.
Current and continuing registration with the Newfoundland and Labrador College of Social Workers (NLCSW) is required.
Minimum of three years of experience within the last five working as a Social Worker in an acute care, long term care or community health care setting is required.
Experience in brief therapy modalities is an asset.
ASIST training is an asset.
Knowledge of quality improvement in health care (i.e. LEAN education and skills and/or demonstrated knowledge of the Model of Improvement).
Knowledge of relevant legislation, including but not limited to, Mental Health Care and Treatment Act, Adult Protection Act, Advanced Health Care Directives Act, Medical Assistance in Dying, and the Children, Youth and Families Act.
Knowledge of mental health and addictions diagnoses and issues, housing and homelessness, healthy living, and health promotion strategies.
Knowledge of discharge planning and community-based resources.
Knowledge of crisis interventions and supportive counseling.
Knowledge of case management.
Demonstrated proficiency in Telus EMR, Meditech and Microsoft Office.
Demonstrated strong clinical leadership skills.
Demonstrated clinical skills in assessment and interventions with individuals, families and groups.
Demonstrated ability to work with a high degree of independence and initiative.
Demonstrated strong program development and evaluation skills.
