Your Opportunity:

The Social Worker will work as a member of the recently established Flow Team within the larger AMH Housing Team. The intention of this team is to provide onsite support and consultation with inpatient teams to expedite and enhance the discharge process which will better support housing outcomes for individuals as they transition from inpatient to community housing. The individuals on this team will have excellent systems knowledge, particularly with AMH Housing resources, community based housing resources and Supportive Living housing options. In addition, these individuals will be well versed in the Strata Pathways referral management system. They will be able to utilize this system to its fullest capacity and also be able to support others to improve their use of the system. This team also currently manages the flow of referrals to the Intake Coordinators office and provides consultation services to a wide variety of referral sources. There are opportunities to provide ongoing education to referral sources on AMH Housing resources and the referral processes.

Required Qualifications:

Completion of bachelor\'s degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW). Additional Required Qualifications:

You will need to be skilled in developing and managing relationships with various stakeholders across the Zone. Your social work skills should include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities. You should bring a vision and a willingness to participate in program planning, development and evaluation. This job will require clear and effective communication with the Care Manager and members of the team. Your grounding in person centered, recovery-oriented practice will guide you in your work. Completion of Bachelor\'s degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW). 3-5 years\' experience in community Mental Health settings particularly with regards to housing assessment intake and support. Valid BCLS-HCP and NCI (Non Violent Crisis Intervention). Candidates must have working knowledge of recovery-based practice, as well as Truama Informed Care and Harm Reduction approaches. A recognition for the importance of partnerships with community agencies is essential. Preferred Qualifications:

Knowledge of and ability to respond to diverse cultural needs of a wide variety of populations. Excellent communication skills. Should be able to work independently, as well as part of a team.
