The Assertive Community Treatment (ACT) Social Worker II is a member of an interdisciplinary team, providing assertive, client focused, culturally appropriate, community based, and recovery-oriented case management to adults experiencing severe and persistent mental illness. Clinical services are most often delivered off-site and targeted to those individuals whose illness presents a barrier to engagement in treatment. Services are most often delivered in the client\'s community and in partnership and collaboration with clients, families, other AHS providers, inner city shelters, agencies, and programs. This position provides additional support to clients in 6 transitional beds partnered with EPIN e4C. The SWII works intensely with these clients, communities and various stakeholders in addressing housing and psychosocial barriers experienced by intensive services clients.

Required Qualifications: Completion of bachelor\'s degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW). Additional Required Qualifications: Ability to work well in a team and seek consultation, when necessary, must be demonstrated. Advanced knowledge and skill in the areas of evidence-based assessment and treatment of mental illness is required. Critical thinking skills, working knowledge on the management of concurrent disorders and medical co-morbidities, and the principles of chronic disease management and recovery are required. Advanced knowledge and skill in the areas of evidence-based assessment and treatment of mental illness is required. Effective interviewing, assessment, psychotherapy, case management and crisis intervention skills are necessary. Current registration with the appropriate professional college/association and current Heart and Stroke Foundation BCLS Level C- Health Care Provider is required. Preferred Qualifications: Minimum education is of a degree preferred. A minimum of 1 year mental health experience is required, preferably in a community or crisis/emergency setting with a year of ACT like experience preferred. Strong understanding of client resources in the community and knowledge of ACT, concurrent disorders.
