This position is part of the centralized Rehabilitation and Allied Health Program responsible for the adult patient population and will support patients specifically who are within the neurological and movement disorders acute care team, concentrating services with the 4A4, 4A3, 4G5, and 5G3 service areas, but will also have opportunities to learn and provide services to other clinical areas on the hospital campus as indicated. Description:

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW). Additional Required Qualifications:

The candidate will have proven communication skills to work cooperatively and effectively with the health care team members, patients and their families in a fast-paced environment with 1-2 years of acute/ambulatory interdisciplinary social work experience. The candidate will hold working knowledge of the health care system and resources. Strong professional judgment, initiative, and effective time management skills are necessary. Current Basic Life Support (BLS-HCP) certification is required. Your quality of work must meet departmental and organizational expectations. Demonstrated clarity in your communication, organization and consistency of ideas, both verbally and written, including clinical documentation, is required. You must be highly motivated, confident, and enthusiastic individual with a positive attitude whose work aligns with AHS Values. You are a reliable worker who uses time effectively and is able to prioritize and complete assigned tasks. You have effective teaching skills for students and other professionals. Preferred Qualifications:

Recent experience in acute care. Experience working with inpatients and ambulatory patients with chronic disease. Neurological sciences experience preferred. Demonstrated ability to foster collaborative partnerships and relationships. Proficiency with Microsoft office including Word, Excel, Outlook and PowerPoint are preferred. Familiarity with Netcare, Connect Care and or principles of electronic documentation are preferred. Master's Degree is an asset.
