Your Opportunity:Located in N.E. Calgary, the Peter Lougheed Centre (PLC) is an acute care hospital offering both inpatient and outpatient services. The PLC has over 600 inpatient beds, including Medical, Surgical, Pediatrics, Women\'s Health and Critical Care. The PLC is the Vascular Centre for Southern Alberta, Southwestern Saskatchewan and Southeastern British Columbia. The Allied Health Department at the PLC is comprised of Social Work, Rehabilitation, and Spiritual Care staff. These staff work as vital members of the interdisciplinary team to provide patient and family-centered care throughout the hospital. Under general supervision from the Clinical Leader and Allied Health management, the successful candidate will provide Social Work services to the Mat/Child and NICU acute care units at the PLC. These services are primarily clinical in nature, and include psychosocial assessments, counseling, case management and discharge planning. This caseload may change based on operational needs. In addition, support to other patient care units will be required to manage daily workload across the Social Work team. This position requires the ability to work independently in a fast-paced environment and yet function as part of the interdisciplinary team. At all times the Social Worker ensures he/she is compliant with ACSW standards of practice.Description:As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
