Your Opportunity:

The Family Care Clinic is known for its innovative approach to providing comprehensive primary care for people with complex needs. This could be any combination of chronic illnesses, mental health difficulties and addiction along with social, economic or cultural factors that negatively impact health. The Clinic is located in the East Calgary Health Centre. The Social Worker II will demonstrate commitment to collaborative practice to enhance patient health outcomes; ability to work independently within a complex and rapidly changing environment; respect for diversity using a supportive, non-judgmental approach; conflict resolution, problem solving, critical thinking and leadership skills are all essential to succeed. This position has minimal Saturday shifts in rotation. Key Responsibilities: Conducts assessments, identifies client issues, formulates and carries out appropriate social work treatment plans in order to meet client needs. Build relationships and provides interventions to clients and families faced with multi-faceted health and social challenges. Facilitates and coordinates individual, group and/or family support or conferences, often involving interdisciplinary teams and/or external stakeholders. Provides crisis support, mediation and conflict resolution strategies. Provides supportive counselling, (the SW II position does not provide therapy). Advocates for clients by coordinating the efforts of all internal and external parties to meet client and family psychosocial and instrumental needs such as housing, food, transportation, income support and financial management. Develops and follows up with discharge and aftercare plans. Promotes relationships and effective liaison with agencies and stakeholders, and in some cases, promotes community awareness of social work or health issues.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW). Additional Required Qualifications:

Minimum of 2 years' experience working as a registered Social Worker. Minimum of 5 years recent experience working with at risk families providing interventions and support around intrinsic needs. Experience with community resources and supporting clients in applying for community supports. Experience working in a community setting with vulnerable populations. Experience in working from a trauma informed lens. Demonstrated ability to work with individuals from diverse backgrounds and cultures using a supportive, non-judgmental approach. Demonstrated ability to collaborate effectively with various community partners. Demonstrated ability to function in a collaborative interdisciplinary environment. Demonstrated ability to use Electronic Medical Record (EMR) and overall computer literacy required. Preferred Qualifications:

Experience working in a primary care setting. Experience working with interdisciplinary team. Experience with adult education and group facilitation. Demonstrated flexibility and adaptability within a complex and rapidly changing environment and may involve working with clients who present with criminal or suicidal tendencies; potential exposure to violence and aggression.
