Your Opportunity:

We are seeking a Social Worker II with strong affinity toward person-centered care practices to join our team. The Social Worker II- Intensive Home Care Team, will have the opportunity to work within a unique and diverse environment and enjoy the benefits of a larger team while collaborating with other professionals. As part of a team, you will support the transition experience of clients/families in the acute care system and in community from the time of referral to placement, ensuring financial, legal, and supportive documents are in place in preparation for upcoming transitions. In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Required Qualifications:

Completion of bachelor\'s degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW). Additional Required Qualifications:

Current BLS-HCP certification. Minimum 2 years\' experience in the clinical area of medical social work. Specific experience in the homecare environment is essential. Excellent communication skills written and verbal. Ability to establish positive, trusting working relationships with a wide variety of clients and families. Ability to organize and set priorities. Considerable initiative and judgement. Experience working within an interdisciplinary team. Demonstrated computer proficiency. Preferred Qualifications:

Meditech/Connect Care and Care Manager training. Demonstrated ability to foster collaborative partnerships and relationships.
