Key Responsibilities
Develop individualized housing and service plans, provide crisis intervention, and offer consistent case management.
Assist clients in navigating subsidized and affordable housing options, completing applications, and understanding tenancy rights and responsibilities.
Conduct home and community visits to assess housing conditions, identify risks, and provide ongoing support.
Advocate for client access to housing services, subsidies, and community supports by collaborating with housing providers, government agencies, and partner organizations.
Build and maintain strong relationships with external service providers, community partners, and internal teams to support holistic care.
Refer clients to appropriate in-house supports (e.g., mental health, employment) and external resources to meet broader needs.
Maintain accurate and timely documentation in accordance with agency policies and privacy legislation.
Adhere to all applicable legislation, regulations, and standards of the Ministry and JF&CS.
Perform other related duties as assigned.
What You Will Bring:
Bachelor of Social Work (BSW) or equivalent education
Registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
2-3 years of clinical or related experience, preferably in housing or homelessness services
Strong skills in assessment, case management, and client-centered service delivery
Experience working within anti-oppressive, trauma-informed, and Housing First frameworks
Knowledge of housing systems, social services, and community resources
Comprehension of French or another language will be considered an asset
