We opened our new Ken and Marilyn Thomson Patient Care Centre in early 2023 providing eight floors of modern and bright care spaces and will continue to transform our campus in the coming years. This is an exciting opportunity for an innovative and experienced Social Worker to join the Extensive Needs Service, a new program for individuals up to the age of 18, whose needs cannot be met in a single service stream and who require an integrated approach across Mental, Behavioural and Developmental Health. This new service model will serve important functions for integration and continuity of service for these high-needs individuals and their families, and will require development and fostering of strong partnerships with other organizations providing services to this population. While this is primarily a clinical role, there will be opportunity to work with other members of the team to develop and implement the social work role based on best practices and evidence. As an integral member of the multidisciplinary team, the Social Worker will provide trauma-informed and compassionate assessment and intervention to children, youth and families with extensive needs such as co-occurring medical, physical and developmental complexities and/or social vulnerabilities.
EDUCATION

  • Master\xe2\x80\x99s degree from an accredited school of Social Work, or equivalent degree, as recognized by the Ontario College of Social Workers and Social Service Workers and/or the Ontario College of Psychotherapists.
  • Member of the Ontario College of Social Workers and Social Service Workers, Ontario College of Psychotherapists, or similar and appropriate regulatory body.


QUALIFICATIONS
  • Minimum 1-3 years recent experience working children and youth with neurodevelopmental conditions, acquired brain injury, and/or mental health conditions.
  • Experience with children and youth in an acute crisis and\\or suffering from a serious mental health illness.
  • Demonstrated recent experience with individual, family and group psychotherapy.
  • Experience with crisis interventions.
  • Experience working with children and youth with histories of severe trauma, abuse and or self-harm.
  • Experience providing psychoeducation around mental illness with children, youth, and families.
  • Demonstrated knowledge of related legislation.
  • Experience with case management principles.
  • Knowledge of child and youth mental health and neurodevelopmental community resources.
  • Demonstrated sensitivity and competency with diverse populations.
  • Demonstrated ability to manage fluctuating clinical responsibilities effectively and efficiently.
  • Demonstrated ability to communicate effectively and in a solution-focused manner with patients, families and members of the interprofessional care team.
  • Good work and attendance record required.
