Reporting to the Manager of the Women & Children\'s Program and working as an active member of the inter-professional team in an inpatient and outpatient setting, the Social Worker will provide a range of access to community resources and services including assessment, crisis intervention, counselling and comprehensive discharge planning with individuals and families. Key components of the role include counselling, conducting psychosocial assessments and providing interventions based on a variety of issues, and should have a keen interest or experience in caring for patient living with sickle cell disease and/or gender affirmation Actively participating in regular team meetings, liaising with community-based providers, advocating on behalf of clients and their families and participation in inter-professional patient and family conferences as needed. The social worker acts as a valued member of the interdisciplinary team in order to see high risk prenatal patients, inpatients/outpatients and follow up post discharge as required.
