Reporting to the Manager of the Women & Children\'s Program and working as an active member of the inter-professional team in an inpatient and outpatient setting, the Social Worker will provide a range of access to community resources and services including assessment, crisis intervention, counselling and comprehensive discharge planning with individuals and families. Key components of the role include counselling, conducting psychosocial assessments and providing interventions based on a variety of issues, and should have a keen interest or experience in caring for patient living with sickle cell disease and/or gender affirmation Actively participating in regular team meetings, liaising with community-based providers, advocating on behalf of clients and their families and participation in inter-professional patient and family conferences as needed. The social worker acts as a valued member of the interdisciplinary team in order to see high risk prenatal patients, inpatients/outpatients and follow up post discharge as required.

Requirements:

  • Graduate from a recognized Master\xe2\x80\xb2s of Social Work Program
  • Registered with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) in good standing
  • Minimum 1-year prior experience in an acute care hospital setting
  • Must have keen interest or experience in Sickle Cell Disease patient population
  • Must have keen interest or experience in Gender Affirmation patient population
  • Demonstrated knowledge of and experience with child protection issues
  • Experience in individual and family counselling
  • Experience working with teens and their families
  • Experience in working with perinatal/neonatal loss and bereavement
  • Experience with mental health issues, particularly related to pregnancy and childcare
  • Excellent knowledge of community resources and liaison/referral experience
  • Experience utilizing online documentation systems, Outlook, Excel
  • Proven excellent interpersonal and communication skills, collaborating and negotiating effectively to achieve desired outcomes during the patient admission/discharge process
  • Proven ability to work effectively as a member of a multidisciplinary team and independently
  • Demonstrates a commitment to customer service when dealing with patients, colleagues, volunteers and other members of the community and public
  • Demonstrates values consistent with SHN\xe2\x80\xb2s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation.
  • Utilize protective equipment provided and identify and reports hazards
  • Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
  • Excellent work performance and attendance
