We are an organization where individuals\' contributions are valued and there are many opportunities for personal growth and development.Position SummaryThe Social Worker provides comprehensive assessment, care planning and treatment coordination for designated clients and their families/ support systems where the social, economic, emotional or psychological circumstances presents barriers to care and or optimum level of functioning. Working collaboratively with members of the patient flow team, other health care providers and the inter-disciplinary team, the Social Worker also provides comprehensive discharge planning services supporting care coordination, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating patient self-management. May be assigned to work in other areas of the hospital as required.Within the acute patient care settings (including Soldiers 1, ICU, Emergency, and surgery) this individual provides clinical social work consultations case management, at times may be required to support transitions/discharges for complex psycho-social situations. Within the Integrated Medical Rehab Service (IMRS) this individual will provide clinical social work consultations / case management as well as discharge planning services. Key to this role is effective communication and collaboration with patient flow team members, physicians and unit team members within the IMRS program.Qualifications

  • BSW required, MSW preferred.
  • Current full registration/membership in the Ontario College of Social Workers and Social Service Workers (OCSWSSW) required.
  • Two (2) years\' experience working with an inter-disciplinary team in a health care setting required.
  • Two (2) to five (5) years\' experience within an acute care hospital setting required.
  • Experience providing scope of practice social work to patients in acute care including Acute Medicine, Acute Surgery, Integrated Medicine and Rehab Services, Emergency and Intensive Care.

  • Demonstrated commitment to a team focused approach within your daily work.
  • Demonstrated case management knowledge, skill and experience with a focus on complex case situations, includes discharge planning skills and activities across all programs.
  • Strong knowledge of community resources and demonstrated ability to work collaboratively and effectively with community partners.
  • Previous experience working with the Public Guardian and Trustee Office
  • Demonstrated excellence in interview skills and ability to provide comprehensive and timely biopsycho-social assessments.
  • Demonstrate comprehensive counseling skills, including crisis counseling, brief therapeutic interventions and methodologies, and loss and grief counselling.
  • Knowledge of and the ability use standardized screening and assessment tools (GDS, DASS, Addictions.)
  • Cognitive Behavior Therapy, Brief Solution Focused therapy, and strengths based experience preferred.
  • Knowledgeable of municipal, provincial, federal income /benefit programs.
  • Demonstrates knowledge and skill in conflict resolution and negotiation.
  • Ability to communicate and negotiate with all levels of the organization and community with documented information and recommended solutions. Ability to identify coordinate utilize and develop community resources.
  • Demonstrate a comprehensive knowledge and understanding of emotional and mental health issues related to illness and injury and patient/family cultural dynamics.
  • Demonstrated ability to organize, prioritize and coordinate workload in a complex environment.
  • Work independently and as a member of, and/or provide leadership to, an interdisciplinary health care team.
  • Demonstrated ability to maintain an evidence-based, best practice approach.
  • Demonstrates commitment to continued professional and personal growth and development through Continuing Education Programs and Self Directed Learning Opportunities.
  • Excellent communication skills, verbal and written with good computer literacy skills .
  • Proven work performance and excellent attendance required.
