This is a full-time, temporary contract position for approximately 6 months.

PURPOSE

:

The Social Worker supports and fosters a holistic and person-centered approach that includes advocacy, support, education, communication and liaising with residents, care partners and staff. The Social Worker works collaboratively with the Manager of Cognitive Care, Behaviour Supports Ontario (BSO) Program, and other disciplines, teams and staff to ensure the mental health and welfare of each resident is addressed. The role strives to promote an optimal quality of life for residents and ensures that they are treated with dignity and respect, having their rights honoured.

MAIN RESPONSIBILITIES:



Acts as the advocate for resident mental health in collaboration with a multi-disciplinary team from a Social Work perspective. Provides education, coaching, counselling, and care for residents, families, care partners and staff regarding mental health and well-being. Adheres to Trinity Village/LHKW's mission, vision, and values. Ensure adherence to: Eden Alternative Philosophy of Care, Green Environmental Initiatives, and CARF Accreditation standards. Maintain attitudes, behaviours, and actions that ensure the safety of residents, staff, and visitors at all times. Be familiar with and adhere to the Resident's Bill of Rights as outlined by the Long-Term Care Homes Act. Remain compliant to all legislative bodies as required by Trinity Village/LHKW for the role of Social Worker. Maintain professional standards and certification as required for the role of Social Worker

DUTIES AND TASKS:



Provide psychosocial knowledge and support to residents, families, care partners and staff as it pertains to all aspects of resident care Counsel residents and families through various sources of stress, such as adjusting to long-term care, changes in health, loss, transitions, etc. Provide ongoing assessment of need, counseling, mediation, and advocacy. Assist in the development of communication strategies for residents and their families and care partners. Actively engage with outside agencies and community groups as resources to offer supports to residents. Liaise on behalf of residents as it pertains to issues related to Powers of Attorney, Substitute Decision Makers, The Consent and Capacity Board, the Public Guardian and Trustee and/or determination of Decisional Capacity. Staff liaison and support to the Family Council, as needed. Visiting volunteer liaison, as needed in collaboration with Volunteer Coordinator. Support to on-site or telemedicine Geriatric Clinics, Geriatric Psychiatry Clinics and other clinics as needed. Facilitate Caregiver Support Groups and training or education sessions. Provide scheduled tours and/or responds to inquiries of the home and educate. residents, families, and members of the outside community regarding the home and services. Member of the Pain and Palliative Care, Behaviour Support Ontario, Diversity and Inclusion, Ethics Committees, PAC and other as needed, including departmental and other meetings as required. Attend initial, annual, and as-needed care conferences to provide Social Work updates and consultation. Administer, interpret and report on the annual Resident & Family Wellness Surveys. Supervise Social Work student, if applicable. Help to support resident admissions and transitions to long-term care home, throughout the health care system, and discharges to the community. Implements evidence-based best practices in treating dementia, delirium, mental health issues, neurological conditions and/or substance use. Has awareness of the effect these issues have on persons, families, and caregivers. Maintain detailed documentation using electronic medical records system including the development of comprehensive plans of care. Administers and interprets comprehensive screening tools and assessments including initial psychosocial assessment, cognitive assessments, depression screenings, etc. Work with Spiritual Care Coordinator to provide psychosocial and practical support at end-of-life to residents and families.

EDUCATION/EXPERIENCE:



Bachelor's or Master's Degree in Social Work required Must be a registered member in good standing with the Ontario College of Social Workers and Social Service Workers under the Social Work and Social Services Work Act, 1998 as required by the Ministry of Health and Long-Term Care. Certificate in Dementia Care or Gerontology preferred Gentle Persuasion Approach preferred PIECES training preferred Validation Therapy Coach and/or Trainer Certification in Positive Approach to CareTM preferred Mental Health First Aid preferred Minimum 2 years experience with clients in long-term care and/or community living with dementia required Minimum six (6) months experience in non-pharmacological approaches to dementia required Experience with PCC, and RAI MDS preferred
