The Social Supports Coordinator provides direction and case management in delivering a range of specific supports and services to eligible persons and families. The programs to which the Supports Coordinator would be responsible to deliver are: Social Assistance; Child Care Subsidy; and Seniors Independence. The Social Supports Coordinator will manage a caseload that includes strong support for a person-centered planning approach related to case management. This approach is based on client empowerment utilizing motivational interviewing as a collaborative guide to elicit and strengthen a persons motivation for change.
Duties will include but are not limited to:

  • Collaborating with the program eligible individual, their family and their support network to ensure that the case plan focuses on maximizing the clients opportunities for full citizenship;
  • Providing screening and initial application supports for any and all clients who may require an eligibility assessment;
  • Completing home and facility visits to promote the person-centered approach that is fundamental to supportive service provision;
  • Contributing to a psychologically safe work environment in a supportive, positive manner;
  • Providing specialization in the assessment and delivery of services utilizing a collaborative case management model with knowledge and expertise on the support needs of people with multiple barriers and working within a multi-disciplinary division;
  • Facilitating the development of personalized services and support plans to individuals and families requiring Social Assistance; Child Care Subsidy or Seniors Independence;
  • Implementing person-centered collaborative case management by combining efficiency, commitment, care and compassion;
  • Determining eligibility in accordance with Program Act/Regulation; policies and procedures;
  • Providing leadership in the development and implementation of support plans designed to identify and address complex support needs for individuals and families at risk;
  • Analysing information presented to confirm accuracy and evaluate the need for appropriate follow up in determining eligibility for service including closure and/or denial of service;
  • Utilizing the motivational interviewing approach which is a conversation style for strengthening a persons own motivation and commitment. It is a strengths based approach that is directional toward change, guiding someone to support identified change goals;
  • Communicating effectively with clients and/or families; the general public; federal; and provincial government; non-government organizations; service providers; funding agencies ( i.e. WCB, CPP, insurance providers, employers, etc.);
  • Knowing all policies and procedures involved in providing a high quality service to clients in the areas of: Social Assistance; Child Care Subsidy and Seniors Independence Initiative;
  • Applying risk management approach in order to recognize the potential for problematic and volatile situations by taking measures to avoid crisis situations; and
  • Other duties as required.
Minimum Qualifications:
  • Must have a university degree in a discipline related to the duties of the position such as Social Sciences.
  • Experience working with individuals and their families in the areas of social services.
  • Experience working in Social Programs, including interpreting related legislation, delivering programs and using ISM, is preferred.
. * Collaborative case management certificate through a recognized program is preferred and experience and knowledge of motivational interviewing is considered an asset.
  • Must become proficient in motivational interviewing through onsite training within 2 years.
  • Must obtain a certificate in collaborative case management though a recognized program within 2 years.
  • Leadership and analytical skills that support innovative and autonomous decision making are considered an asset.
  • Demonstrated expertise in complex health and social needs planning is considered an asset.
  • Experience with a variety of software programs ie. Group Wise, and Word.
  • Strong knowledge and understanding of community, provincial and federal resources and navigational principles.
  • Excellent written and oral communication skills, as well as excellent interpersonal, time management, problem solving, organizational skills and ability to work independently with minimal supervision.
