Conditions of Employment:


Must be legally entitled to work in Canada Must be able to provide and maintain a satisfactory Criminal Record Check with Vulnerable Sector Search. Must have a valid full stage Class 5F driver's licence, access to a vehicle and the ability to travel within the Province of Manitoba for government business purposes. Must be able to provide and maintain a satisfactory Child Abuse Registry Check Must be able to provide and maintain a satisfactory Adult Abuse Registry Check Must be available to work overtime when required.

Qualifications:


Essential:




Post-secondary education in the social services field. This may include a certificate, diploma or degree. An equivalent combination of education, training and experience within the social services field may be considered. Case management experience in an integrated services environment that includes brief counselling, conducting needs assessments, service planning and coordination, resource development and follow-up. Experience working with individuals and families experiencing housing insecurity, providing supports based on the individual needs of the participants. Experience with health system navigation that includes information sharing, referrals, advocacy and collaborative planning. Strong verbal communication skills with the ability to effectively communicate or deliver difficult messages or information to applicants and partners. Effective written communication skills to write clear and concise correspondence to a variety of audiences. Strong interpersonal skills including the ability to interact calmly, sensitively, respectfully and in a non-judgmental manner to resolve conflict effectively with diverse stakeholders. Excellent time-management and organizational skills including with the ability to manage high-volume requests and competing demands within a complex and fast-paced work environment. Experience participating in collaborative planning with other government agencies, health services and community agencies.

Knowledge of a variety of housing models available to Manitobans Knowledge of community resources in the areas of housing and homelessness, older adults, mental health, and substance use.

Duties:


Reporting to a Program Manager, the Social Services Case Manager (SSCM) provides services to applicants to the Social Housing Rental Program who are facing complex barriers to housing. The incumbent is responsible for liaising with community and health organizations, needs assessments and the review and development of person-centred plans in support of safe and stable tenancies. The SSCM participates in critical applications/intake process, sector committees and networks, crisis response and departmental initiatives. The SSCM takes a collaborative approach to all job duties and is a vital link between the branch and the broader health and social services sector.
