Full Time

Salary Range: $31.74 - $37.35 /hour

St. Joseph's Family Medical Centre continues to expand its role in the treatment of complex medical and chronic disease illness prevention, research and education. Our primary care teams specialize in the treatment of complex and chronic disease conditions with the goal of assisting individuals to reach optimal health and well-being. Our different teams work together to provide patients with comprehensive assessment diagnoses, disease prevention and management strategies and follow up care.




Our team provides care, support and education for individuals diagnosed with chronic diseases, who are dealing with various challenges that make managing their healthcare needs more difficult, such as poverty, language barriers, social isolation, addictions or mental illness.




The Social Service Worker supports patients to optimize their self-care capacity and self-care resources related to housing, food security and assistive devices. The Social Service Worker will assist in navigating the many systems and resources available to the patient. They will work in collaboration with members of the Primary Care Diabetes Support Program and the Family Medicine Team.









Essential Qualifications



Social Service Diploma from an accredited Community College Valid registration with the Ontario College of Social Workers and Social Service Workers 1 year experience in social service work practice in hospital or long-term care, with expertise in the care of complex and chronic health conditions Demonstrated strength in written/verbal communication and case management Experience working as part of an interdisciplinary team Must be capable of establishing positive interpersonal relationships and able to work as an effective team member Ability to maintain professionalism, discretion, and respect confidentiality always Demonstrated skills in providing education to patients, families and the health care team Proficient computer skills with a variety of programs Knowledge of relevant legislation including the Health Care Consent Act and Substitute Decisions Act Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA

Preferred Qualifications

Proficiency in French would be an asset

Teaching and Research

St Joseph's Health Care London through its affiliation with Western University and Fanshawe College is a leading research and teaching hospital. As an employee of St Joseph's, you will be expected to engage in role related teaching and research activities in addition to any of your clinical duties

Immunization Requirements

Provide vaccination records or proof of immunity against measles, mumps rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza. Provide documentation of the Tuberculosis skin testing
