Job Description
Primacare Living Solutions is currently looking to fill a role of a social worker for our Long-Term Care Home in London Responsibilities: - Providing Support and Counselling to Residents and Families - Facilitating admissions, discharges and transfers - Facilitating tours of the home for new residents and families - Attending and participating in Care Conferences - Work with all members of the interdisciplinary team Qualifications - Degree in social work - Membership in the Ontario College of Social Workers - Experience working in a similar or same role in Long Term Care - Knowledge of applicable regulations Job Type: Permanent Salary: $51,000.00 per year Schedule:
- 8 hour shift
- Social Service work in LTC: 2 years (preferred)
- Ontario College of Social Workers (preferred)
