Location: Owen Sound, Ontario.

Position/Status: Social Service Worker- Permanent - Full-Time

Duties and Responsibilities

\xc2\xb7Complete resident pre-admission, admission and ongoing social work assessment to identify goals and strategies, expected outcomes for care and target dates.

\xc2\xb7Participate in interdisciplinary team conferences as required.

\xc2\xb7Act as liaison, as appropriate, with families.

\xc2\xb7Provide counselling or group sessions for residents and families as identified - i.e., orientation adjustment programs, crisis problem solving, bereavement counselling, etc..

\xc2\xb7Act as a resource person to staff who have identified needs for counselling, referrals or support.

\xc2\xb7Plan, provide or arrange for in-service programs for facility staff, residents and families as identified.

\xc2\xb7Maintain all required records, reports, statistics, etc. for both residents and department in accordance with policies, procedures and legislative requirement.

\xc2\xb7Participate on committees related to resident care and operation of the facility as required.

\xc2\xb7Establishes and maintains communications with other disciplines, residents and families, to ensure resident needs are being met.

\xc2\xb7Liaises and consults with government officials, company resources and professionals in relation to resident care needs.

Qualifications

\xc2\xb7Social Services Worker (SSW) Diploma from an accredited College.

\xc2\xb7Bachelor\'s degree (BSW) in Social Work from an accredited University is an asset

\xc2\xb7Sound knowledge of community resources and services.

\xc2\xb7Sound knowledge in the fields of aging, gerontology and dementia.

\xc2\xb7Previous experience in long term care preferred.

\xc2\xb7Knowledge of applicable legislation, individual counselling and group sessions.

\xc2\xb7Knowledge of occupational health and safety practices, principles and legislation.

\xc2\xb7Ability to read and write English as well as understand complex verbal and written instructions in English.
