POSITION SUMMARY: The role of the Siksika Community Health Nurse is to provide comprehensive health programs, health promotion, prevention education, and support for the community and agencies within Siksika Nation.

Responsibilities include, but will not be limited to:

Assesses plans and implements a comprehensive preventive community health program. Maintain the security, confidentiality, accuracy, integrity and legality of the patient records including the electronic database system. Collaborate, establish and maintain working relationships with, both internal (e.g. First nations leadership, other health care providers) and external (e.g. Provincial and Federal counterparts) health care professionals and community service providers. Records Communicable Disease information for submission to Regional, Federal and Provincial authorities for contact tracing, surveillance, epidemiological research and development of public health policies. Selects appropriate health information, develops health teaching plans and selects appropriate education models and methods of presentation to assist client(s) to form positive attitudes and make decisions related to health, lifestyle or specific conditions. Provides immunization to clients whose ages range from infancy to the elderly, to provide protection from communicable diseases. Assists in the investigation and management of communicable diseases in the community including administering screening tests, collecting specimens, health education, contact tracing, and treatment. Provides community health programs for community members of all ages to target groups at risk to promote health and maintain wellness. This includes pre and post natal clients, infants, children, and adolescents, individuals with long-term illnesses and disabilities, and elders. Services include screening for a variety of risk situations, counselling, referring and education. The work may include providing assistance in the preparation of community disaster plans and facilitating and promoting a team approach in the provision of community health programs. Collaborates with visiting health professionals, such as Environmental Health Officer (EHO), dental therapists, in the treatment of clients and promoting health education to the First Nations community. Operates vehicles to travel to, from and within the community to carry out routine and emergency nursing and administrative services. Notifies management of any maintenance problems to ensure routine maintenance is carried out. Routinely re-stocks supplies and equipment. Assists in carrying out inventory and reordering of medical supplies and processes requests for equipment maintenance. Ensures the maintenance of the security system established for storing and dispensing over the counter and pharmaceutical products (vaccines) used for patient care including the proper reporting in the event of theft or loss. Knowledge of the application of concepts, principles, practices and theories of community health nursing, population health, disease and injury prevention, and health promotion concepts in order to deliver a comprehensive Community Health Nursing program including communicable disease control. Knowledge of pharmacology in order to interpret the adverse reactions. Knowledge of concepts and theories of crisis intervention including mental health assessment and interventions; defusing hostile or emotional charged clients; client referrals and follow-up. Knowledge of facility organization, reporting structures, documentation, record keeping, operations and the roles and responsibilities of employees within the health service is required to participate in the day-to-day operations of the facility and to apply security and emergency procedures. Knowledge of Siksika's infrastructure, politics and protocols to provide culturally sensitive service delivery and develop working relationships. Knowledge of community geography to carry out home visits. Promotes the philosophy of self-care and independence by providing instructions to individuals, families and other groups of people. Mobilizes and coordinates the other health and social resources required to permit clients and their families to function as independently as possible, both as individuals and as a family unit. The nurse organizes/plans, using care mapping, to ensure that the resources work in concert to improve or maintain the clients/family ability to function at the best level possible, both as separate individuals and together as a unit. Establishes and maintains close liaison with families, significant others, attending physicians and other health care providers involved in the client's care. Reviews, reassesses and revises with the client, the family and other health care providers, the client's progress, which may include planning and attending care reviews and client conferences, if required. Plans effectively with the clients and family and other care providers for discharge.

Participates in quality assurance activities as requested.

Informs Team Leader of potential political, media, health and safety issues affecting delivery of the community's health services to ensure the Team Leader is aware of the situation and can take appropriate action to address the issue or issues.

EMPLOYMENT QUALIFICATIONS:

Education:

Graduation from an accredited school of nursing. Current registration with the CRNA

Minimum three years of community health experience preferred.

Current Cardiopulmonary Resuscitation, Basic Rescuer level and First Aid Certificate. Knowledge, Skills and Abilities: Excellent interpersonal and organizational skills. Good verbal and written communication skills Requirements: Class 5 License and own vehicle Must be culturally competent and provide care within the context of Siksika Nation's culture and traditions Moderate knowledge of the philosophy and issues regarding the delivery of First Nation health care
