Exciting Opportunity as JFSC is expanding and looking for a Social Worker with Mental Health and Addictions experience.

Primary Duties and Responsibilities:

Complete an initial assessment

Develop a service plan and goals in partnership with the client

Participate in interdisciplinary service planning and conferences by providing the team with current information and observations

Measure clients' progress regularly and adjust services accordingly

Assist with program development and implementation aligning with best practices

Carry out case management functions including convening case conferences, liaising with community services and other professionals (especially education, social services, justice and health)

Ensure the maintenance of accurate records in line with practice policy

Qualifications:

A post-secondary diploma or degree in Human/Social Services, Addictions, Counselling, Psychology, or a related field Minimum 3 years' relevant practical experience working with Seniors with Mental Health and/or Addictions Experience in an intensive case management role serving a vulnerable population, including individuals with trauma Excellent communication, organization, time management, and coordination skills Must have a positive and collaborative attitude, demonstrate flexibility, reliability and show initiative Non-violent crisis intervention and First Aid/CPR training are assets Class 5 driver's license, personal vehicle, clean driver's abstract and $2 million insurance liability Clear Criminal Record Check with Vulnerable Sector dated within three months of hire

Pay: $56,000.00-$62,000.00 per year
