Job Description

The Rexdale Community Health Centre (RCHC) is a multi-service centre providing primary health care, health promotion and community support services, including settlement and community development activities for families, children, youth, adults and seniors of diverse backgrounds.

Rexdale Community Health Centre is looking for a Seniors' Program Coordinator who will coordinate the design, development, implementation and evaluation of RCHC Seniors Programs and Services. As a member of the Centre's multi-disciplinary team, the Program Coordinator for Seniors provides support to the RCHC community with a specific focus on the older adult 55+ priority population.

Responsibilities



Coordinate the long-range planning for the Senior Programs and Special Events. Ensure the program activities are carried out in the spirit of the Centre's mission and goals. Coordinate the development and resource requirements of the program Ensure the participants are involved in decision-making and program planning for all the program activities. Coordinate the implementation of program evaluation in accordance with the format designed by the Centre Assist in the preparation of funding submissions/proposals for the Seniors Programs. Assist in the development of strategies for funding for the Senior's Programs. Ensure program attendance recording is completed for the programs. Ensure that all programs are designed, developed and resourced on a 3-month advanced planning schedule. Ensure program calendars, promotional material and website information are up to date. Ensure the scheduling of Program Instructors and program presenters according to the program's needs. Develop and maintain a filing system. Assists in the supervision of program facilitators, project partners, volunteers and students Gather community resource information and liaise with community agencies and service providers. Ensure there are linkages developed between other related programs and services in the community. Identify programs and resources needed to support the Centre's activities by developing and maintaining informal and formal community networks Develop and maintain contacts with community agencies, local businesses, and appropriate referral agencies. Represent Rexdale Community Health Centre in Inter-agency networks Assist program participants and community members in identifying issues and concerns and developing appropriate action in accordance with the Centre's goals and mission. Encourage program participants to develop links with coalitions and groups that represent their interests to promote individual and group empowerment Publicize the program activities using written material and public speaking Ensure comprehensive service for clients by providing information and, when necessary, referring them to RCHC internal services and other social care agencies and healthcare professionals. Maintain appropriate client records, assessment notes, and correspondence within the safeguard of confidentiality. Where appropriate, provide translation/ interpretation services Other duties as assigned.

Qualifications



University degree and/or College Diploma in a related field; At least 3 years of experience working with seniors of diverse cultures in a community-based setting. Skills in program planning and development Good English language written and verbal skills Knowledge of services and community organizations for seniors of diverse cultures, or the ability to make such connections Understanding of seniors' issues in a culturally diverse setting Demonstrated commitment to social change Advanced Digital literacy skills are an asset Second language writing and verbal skills are an asset Knowledge of the Rexdale community and its agencies Experience working/supervising in an unionized environment Bilingual in French and English, considered a strong asset Must be able to provide a vulnerable sector security clearance

Job Types: Full-time, Fixed term contract
Contract length: 12 months

Pay: $51,836.00-$62,458.00 per year
