SALARY: $37.70 to $45.92 per hour (Subject to employment status)

JOB PURPOSE:

To provide advanced clinical nursing expertise in the assessment, planning, implementation and evaluation of care to patients under the service of Interventional Cardiology, General Cardiology, Electrophysiology (EP), Heart Function Clinic and Access within the New Brunswick Heart Centre. To provide advanced clinical nursing expertise in the triage of inpatient and outpatient referrals and assessment, planning, implementation and evaluation of care to patients under the service of Interventional Cardiology, General Cardiology, Heart Failure Clinic, Access and Electrophysiology (EP) within the New Brunswick Heart Centre. This role oversees patients in both the inpatient and outpatient (clinics) settings along with responsibilities for aspects of Cardiology patient triage and coordination of patient flow through the various NBHC services. The role also includes the provision of educational and emotional support to patients and families within the services. In addition, the Nurse Associate acts as a resource for nursing staff and allied healthcare professionals from internal and external environments.

QUALIFICATIONS:

  • Bachelor's degree in nursing.
  • Current RN registration in the Province of New Brunswick or registration eligibility.
  • Minimum of 5 years current experience in Coronary Care. Previous experience in an advance role, or ICU environment, is a definite asset to this position.
  • Demonstrates competency as a practitioner with demonstrated accuracy and attention to detail which is an essential element in this competency.
  • Demonstrates problem solving and critical thinking skills.
  • Demonstrates advanced skills in physical assessment and interpretation of clinical data.
  • Current certification in BCLS, ACLS and Defibrillation.
  • Proven leadership qualities as demonstrated by initiative, good judgment and the ability to instill confidence in peers.
  • The Nurse Associate must be able to synthesize objective and subjective clinical data and document/communicate in a sequential and logical manner.
  • Ability to work independently and as a part of a multidisciplinary team.
  • Effective communication skills in both English and French. (Bilingual) Provincial testing is mandatory.
  • Demonstrates effective teaching skills.
  • Willing to take any additional training.
  • Excellent previous work and attendance record.
  • Physically able to perform assigned duties.
  • Written and spoken competence in English (B2.1 or Advanced) and spoken competence in French (B1.2 or Intermediate Plus) are required.
KEY RESPONSIBILITIES:
  • Based on established protocols, participates in the access management and sequential transition of the patients through the NBHC system back to referring sites or community.
  • Conducts and documents a comprehensive history and an advanced cardiovascular-focused physical assessment. Maintains up to date progress notes on each of these patients.
  • Reviews clinical findings with the attending physician and initiates appropriate intervention(s), initiating orders, preparing patient, family and nursing staff accordingly.
  • In conjunction with the physician, determines the need for non-invasive and invasive cardiac interventions.
  • Provides patient and family education and follow-up instruction in both official languages (English and French).
  • Coordinate the care of patients with appropriate clinicians, care teams, home health care agencies and hospice providers to achieve desired outcomes using evidence-based information to support directives.
  • Based on established CCS guidelines, is responsible for triaging and assessment of urgency ratings for interventional cardiology referrals as well as the coordination of patient flow through the various NBHC areas.
  • Reviews in-patients daily and in collaboration with the health care team and physician, provides daily case management
  • Communicate with referring physicians and other health care personnel, agencies and institutions in both official languages (English & French) to facilitate timely access to the Interventional cardiology care and diagnostic service and discharge or repatriation to referring facility or physician, as required.
  • Identifies individuals that are at greater risk of experiencing complications from an invasive cardiac intervention procedure and reviews concerns with attending physician.
  • Following established protocols, Initiates pre and post procedure orders (as required).
  • Obtains and documents patient home medications using Medication Reconciliation approach. Following discussion with attending physician electronically enters medications as appropriate (with adjustments) as well as standard medications for pre and post procedures.
  • Collaborates with members of the multidisciplinary team to provide effective and efficient tertiary cardiology service.
  • Collaborates and communicates with the other NB Heart center services to ensure seamless movement of patients on their continuum of care Identifies clinical knowledge needs of internal nurse associate team, and within individual healthcare providers, specific groups or populations and recommends teaching initiatives specific to target audience.
  • Participates in service patient rounds as indicated
  • Participate in general teaching sessions of the multidisciplinary team whenever appropriate.
  • Participate in research projects and program data collection as needed.
  • Participates in the identification, development and delivery of patient/family teaching material and assists in standardizing protocols (standing orders) in the electronics clinical information system.
  • Participates in change projects for improvement of the Interventional cardiology service and the Interventional Associate role when appropriate.
