SALARY: $35.69 to $45.22 per hour

JOB PURPOSE:

Provision of Education and Nursing Care to meet the individual renal patients needs by utilizing the nursing process.

QUALIFICATIONS:

  • Currently registered with the Nurses Association of New Brunswick.
  • Demonstrates & understands the Nursing Process.
  • Shares on-call with other dialysis teaching staff.
  • Certification in Nephrology an asset.
  • Hemodialysis experience required (minimum of one year recent experience)
  • Peritoneal Dialysis experience required (minimum of one year recent experience)
  • Demonstrates excellence attendance record.
  • Proper regard for confidential information.
  • Demonstrates initiative & good judgement.
  • Ability to communicate effectively.
  • Currently holds valid N.B. Driver\'s license.
  • Able to travel to homes across Horizon Network
  • Ability to attend work on a regular basis.
  • Ability to perform the duties of the position.
  • Good work record.
  • Written and spoken competence in English (B2.1 or Advanced) and spoken competence in French (B1.2 or Intermediate Plus) are required.
KEY RESPONSIBILITIES:

REGULAR DUTIES AND/OR RESPONSIBILITIES:
  • Assesses the health care needs of renal patients.
  • Identifies renal patients\' needs & problems utilizing an assessment tool.
  • Identifies needs affecting discharge planning & potential services required at home.
  • Formulates objectives specific to the patient needs i.e.: nursing diagnosis.
  • Provides health teaching, counseling, health promotion to individuals or groups.
  • Uses the Kidney Foundation Manual & appropriate renal teaching tools to effectively prepare the renal patient to manage his/her disease.
  • Provides hemodialysis teaching for home patients utilizing adult learning principles
  • Provides peritoneal dialysis teaching for home patients utilizing adult learning principles
  • Provides teaching for patients awaiting a kidney transplant & their potential donors when applicable. Provide teaching for patients with a kidney transplant to ensure longevity of the transplant regarding adequate follow-up & medication compliance.
  • Provides teaching to patients not on treatment regarding medication & diet management & assists them in treatment choices as appropriate.
  • Provides hemodialysis treatments for relief of helpers.
  • Performs home visit assessments as well as phone support to home dialysis patients and families
  • Provides nursing care required to achieve the patient\'s desired outcomes through regular follow-up.
  • Prepares for, participates and provides follow-up for clinics.
  • Monitors hemodialysis patient
  • Monitors peritoneal dialysis patient
  • Monitors pre-treatment patients; assesses lab results & provides regular follow-up to assist with initiation of treatment when necessary.
  • Evaluates outcomes of nursing actions for further assessment & planning.
  • Uses principles of renal replacement therapy to evaluate outcomes & plan future care & treatment.
  • Utilizes the computerized renal data management system & other forms of composite data collection to help identify trends & respond appropriately.
  • Works within the primary nursing delivery model.
  • Utilizes effective communication skills to achieve a helping relationship.
  • Supports new employees during orientation.
  • Directs & supervises non-professional staff.
  • Interacts with the interdisciplinary team in identifying, planning, implementing, & evaluating changes.
  • Demonstrates initiative for personal professional development.
  • Continues to update renal knowledge through ongoing learning- participation in Journal Clubs, webcasts &conferences.
  • Becomes certified as a Nephrology Nurse; Cneph\xc2\xa9; under the CNA certification program.
  • Participates in programs promoting quality improvement on nursing unit.
  • On-call duties required
