JOB SUMMARY

Works as part of a team to support person-centered care to the residents of the Long Term Care home.



Under the direction of the Director of Resident Care and in keeping with the philosophy, objectives, policies and procedures of Seniors Services, the RPN - Resource works as part of a team to ensure professional nursing care to residents and offers direction and assistance to Personal Support Workers (PSWs) and Registered Practical Nurses (RPNs).



In collaboration with the interdisciplinary team, practice in accordance with the Regulated Health Professions Act, Long-Term Care Homes Act and regulations, Occupational Health & Safety Act, and the standards set by the College of Nurses of Ontario (CNO). Facilitate interdisciplinary, resident-centred care to achieve desired quality of life outcomes and provide professional nursing care to residents.



Under the direction of the Leadership Team of the home, the RPN - Resource provides concentrated nursing support to or acts as a lead for activities such as supplies & inventory, core programs, special projects, meetings and care conferences, peer-to-peer training, audits, documentation, reports, wander management system administration, and narcotic destruction. Acts as a resource and champion on the home for core programs, policies and systems.

In accordance with the schedule and/or Staffing Plan, also required to work on a home area or elsewhere within the long term care home (ex. outbreak, staffing shortage, operational need, etc.).

QUALIFICATIONS

EDUCATION
---------

Completion of grade 12 (or equivalent) Completion of a 2-year Registered Practical Nurse program (or equivalent), as approved by the College of Nurses of Ontario (CNO) Completion of an additional 1 year of education, training or certification in any of the following areas (including a combination): wound care (SWAN), ostomy, continence, falls prevention, IV initiation and maintenance, blood glucose monitoring, skin assessments, interRAI Long-Term Care Facilities (interRAI LTCF), PointClickCare (PCC), geriatrics, dementia care, or other similar, related areas Training in Gentle Persuasive Approaches (GPA), P.I.E.C.E.S. (Physical. Intellectual. Emotional. Capabilities. Environment. Social.), Infection Prevention and Control (IPAC), Positive Approaches to Care (PAC), and DementiAbility, are strongly preferred Current certificate in CPR
KNOWLEDGE/EXPERIENCE

Minimum of 2 years of current, relevant experience (within the last 3 years), Experience in a long-term care or similar setting (ex. congregate care, hospital, community, etc), long-term care experience is strongly preferred Current (within the last 3 years) experience in administration of medications Experience mentoring, coaching or training in a peer-to-peer environment is an asset Knowledge of resident safety issues and demonstrated capacity to apply resident safety practices and required nursing interventions Demonstrated knowledge of the Fixing Long-Term Care Homes Act and Residents' Bill of Rights. Knowledge of the CNO's RPN collaborative model and demonstrated ability to problem solve within the RPN scope of practice Familiarity with the relevant policies and procedures of Seniors Services is strongly preferred

SPECIAL CONDITIONS

Current registration as Registered Practical Nurse with the CNO In accordance with the Corporate Policy and applicable Legislation/Regulation(s), the position requires the incumbent to undergo a Police Record Check and submit a satisfactory Vulnerable Sector Check, as well as complete any subsequent declaration or check as required by legislation/Act, regulation, and/or policy Compliance with any requirements, including immunization and screening measures, of the infection prevention and control program as required by applicable legislation/Act, regulation or policy; some measures are only recommendations (not requirements) + Staff are required to be screened for Tuberculosis in accordance with the applicable legislation/Act
Must have a good attendance record Must be able and available to work shift work, weekends, and statutory holidays (24 x 7 x 365 operation) and meet requirements of availability as per the Collective Agreement. In accordance with the schedule and/or Staffing Plan, also required to work on a home area or elsewhere within the long term care home (ex. outbreak, staffing shortage, operational need, etc.).

Shift Information

Currently scheduled 16 hour days/ evenings shifts, may be required to work days and nights including weekends. You must declare a minimum of 8 shifts (64 hours) per week and commit to working no less than 3 shifts per week (24 hours). It is understood that you will be scheduled to work if the work is available.
