Job Overview


We are seeking a skilled and compassionate Registered Practical Nurse (RPN) who will apply both clinical knowledge and leadership skills to provide direction, assistance and supervision to PSWs/HCAs and support other team members. This role requires a balanced approach that integrates clinical expertise with management capabilities, emphasizing health promotion and disease prevention to enhance resident wellness. Working collaboratively with physicians, the RPN will help develop and implement care plans, assist in rehabilitation, and provide education and emotional support to the residents and their families.

Responsibilities



Follows the College of Nurses' guidelines to teach, assign, and supervise nursing care. Identifies opportunities to educate residents and families based on assessments and evaluates the results. Observes personal care provided by PSWs/HCAs, offers training as needed, and monitors their skills. Keeps nursing knowledge and skills up to date, including long-term care practices, technology, and CPR. Identifies and assesses each resident's individual care needs and develops appropriate care plans to address them. Provides or assigns care when the resident's condition is stable and predictable. Seeks support from an RN when a resident's condition is complex or unpredictable. Offers personal support to residents with dignity and respect. Administers medications and treatments as prescribed by the Home's physician. Ensures resident safety by reporting and documenting unsafe practices, abuse, or misconduct, and initiates investigations as per policy. Coordinates and oversees the implementation and evaluation of resident care plans in collaboration with the multidisciplinary team, utilizing the nursing process.

Qualifications



Current Certificate of Registration in good standing with the College of Nurses of Ontario (CNO) Valid First Aid and CPR certification Valid Vulnerable Sector Check Proof of recent tuberculosis (TB) testing or vaccination Knowledge of RAI-MDS Experience in geriatrics and LTC facility is an asset Strong communication skills, both verbal and written
