Overview

We are seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our team. The ideal candidate will possess a strong background in Long Term Care.

Responsibilities

Deliver direct patient care with a focus on comfort and well-being. Administer medications and treatments as prescribed by physicians. Monitor patient vital signs and report any changes in condition. Provide support for patients with acute care needs, dementia, and pain management. Assist in case management and coordinate care plans with healthcare teams. Maintain accurate patient records using electronic health record systems such as Point Click Care Educate patients and families on health management and medication usage. Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Requirements

Valid Registered Practical Nurse license from CNO Nursing home experience preferred Familiarity with chronic health conditions and dementia. Proficient in medical terminology and medication administration techniques. Experience with electronic health records is desirable. Strong communication skills and the ability to work effectively within a team environment. Compassionate approach to patient care with an emphasis on empathy and understanding.
Job Types: Part-time, Permanent

Pay: $27.27-$43.38 per hour

Expected hours: 30 per week
