Overview


We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) specializing in Harm Reduction to join our team. The ideal candidate will play a crucial role in providing high-quality patient care to individuals facing substance use challenges. This position requires a strong commitment to promoting health and well-being while ensuring a safe and supportive environment for all patients.

Responsibilities



Deliver comprehensive nursing care to patients in accordance with established harm reduction principles. Administer medications and monitor patient responses, ensuring adherence to MAT & OAT management protocols. Conduct thorough assessments of patients' physical and emotional health, documenting findings accurately. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient's needs. Provide education and support to patients and their families regarding harm reduction strategies and available resources. Assist with laboratory procedures, including urine drug screening, phlebotomy, as required for patient assessments. Maintain accurate patient records and ensure compliance with all regulatory standards within clinic. Participate in ongoing professional development opportunities to enhance skills related to addictions and mental health.

Qualifications



Current registration as a Practical Nurse with the relevant licensing body. Dispensing medication Health teaching surrounding side effects and how to manage the symptoms that interfere with quality of life. Transcription of orders and maintenance of medication records. Monitoring compliance to the care plans. Assist individuals with personal hygiene when necessary. Demonstrate harm reduction and recovery oriented practice (ROP) philosophy and practices within daily work and interactions Intervene to crisis as necessary and work to intervene before a crisis escalates and provide appropriate follow-up. Participates in the safety of the program participants at all times. Strong knowledge of medication administration practices and patient care protocols. Familiarity with harm reduction strategies and their application in nursing practice is an asset. Excellent communication skills, both verbal and written, with the ability to build rapport with patients from diverse backgrounds. Ability to work collaboratively within a multidisciplinary team while demonstrating strong organizational skills. Previous experience in acute care or nursing home settings is advantageous. Certification in phlebotomy is a plus but not mandatory.
