We are seeking a compassionate and dedicated Home Care Nurse to provide leadership to a team of HCA's. High-quality patient care in the comfort of our clients' homes. The ideal candidate will have a strong background in nursing, scheduling, Client assessment, problem-solving, and intake assessment. This role requires excellent communication skills, problem-solving, and a commitment to enhancing the quality of life for patients.

Responsibilities



Conduct comprehensive assessments of patients' health status and develop individualized care plans. Administer medications and treatments as prescribed by physicians. Monitor patients' vital signs and overall health, reporting any changes to the healthcare team. Adjusting and planning schedules as needed Provide education and support to patients and their families regarding health management and wellness. Assist with and oversee the delivery of, daily living activities, including personal hygiene, mobility, and nutrition. Maintain accurate medical records and documentation in compliance with healthcare regulations. Collaborate with other healthcare professionals to ensure coordinated care for patients.

Skills



Strong knowledge of patient care practices and medical terminology. Excellent interpersonal skills with the ability to build rapport with patients and families. Proficient in nursing procedures and techniques, demonstrating a high level of clinical competency. Ability to work independently while managing time effectively to meet patient needs. Compassionate attitude towards patient care, ensuring dignity and respect at all times. Strong problem-solving skills, with the ability to adapt care plans as necessary based on patient conditions.
Job Type: Full-time

Pay: $33.46-$40.00 per hour
