As the Continuing Care/Home Care Manager, you will provide effective leadership and strategic direction to an interdisciplinary team of healthcare professionals in both the home care and Type A/B care departments. Your role will focus on delivering safe, high-quality, patient- and family-centred services. Key responsibilities include: Leading, mentoring, and supporting clinical teams to foster excellence in service delivery. Making informed frontline decisions that prioritize quality care and staff engagement. Overseeing compliance with continuing care regulations, performance standards, and operational accountabilities. Coordinating and collaborating with physicians, allied health professionals, and community partners to ensure continuity of care.
