Job Summary


As part of the Harvest House Atlantic Housing Team, the Rising Tide Housing Support Case Worker will support tenants in permanent housing units to develop tenancy skills and achieve life goals through action-oriented case management.

To ensure that HHA continues in its leadership role in community for quality systems and data management, case management activities and delivery on goods and service outcomes will be tracked in the appropriate modules of Hifis 4.

Duties



Complete a move in assessment tool to identify housing needs, food security needs, continuity of care plan for any health and mental health & addiction service needs and support tenant to facilitate any action items identified. Through provision of client-led support, guide the tenant to establish a successful tenancy through meeting of immediate needs, creating community connection, and assisting with community navigation, as required. Respond to tenant crisis, as required, through strengths-based, trauma- informed approaches Use appropriate assessment tools, case management resources, and readiness rulers to assess and support a client-led action-oriented case plan to achieve client life goals. Engage with community partners to better serve tenants. Maintain all Hifis 4 files in appropriate modules. Attend mandatory professional development and recertification programs as scheduled.

Pre-requisites:



Valid driver's license with driver abstract provided Reliable vehicle Proof of insurance Vulnerable Sector Check

Experience:



Previous experience working with homeless populations or individuals facing housing instability. Case management experience, especially in social services, shelters, or supportive housing programs. Experience navigating housing programs like Section 8, HUD, or other rental assistance services. Diploma course related to housing and social services

Pay: $41,600.00-$49,920.00 per year
