ABOUT THE ROLE




This position is responsible for supporting and assisting in restorative care program and opportunities to meet the social, emotional, spiritual, physical, and restorative care needs of all residents. In collaboration with the contracted physiotherapist, the Restorative Care Coordinator and other professionals, this position is responsible for supporting the implementation, evaluation, and revision of the therapeutic plan of care for residents for the restorative care program.


WHAT YOU'LL BE DOING




As per the Resident Bill of Rights, supports the residents participating in the program receive care and assistance towards independence based on a restorative care philosophy to maximize autonomy. Delivers and supports the restorative care approaches to ensure they are integrated in the plan of care provided to all residents. Implements and supports restorative care approaches to ensure the resident can maintain or improve their functional capacities in various aspects of daily living including personal hygiene, restorative eating, restorative walking, restorative transfers, bed mobility, toileting, dressing and grooming. Monitors the behaviour and condition of residents during programs and reports any significant changes to the Restorative Care Coordinator/designate and nursing staff as appropriate. Documents the restorative plan of care including progress notes and residents needs, and preferences in a timely manner. Supports the Restorative Care Coordinator to ensure that the resident and the resident's substitute decision-maker are consulted in the development and implementation of the restorative plan of care. Assists and supports residents' daily restorative plan of care and ensuring progress are documented and implemented in a timely manner. Tracks and documents resident's attendance and participation in the Restorative Care Program. Delivers interventions outlined in Restorative Plan of Care. Assists and supports the delivery a variety of training/re-training, stretching, strengthening, and active and passive range of motion, activities of daily living, mobility exercises to residents at structured times. Collaborates with Occupational Therapist to ensure residents are properly using their assistive devices and sitting posture. Assists in liaising with nursing and therapy services to ensure timely communication between program areas. Provides residents with a safe and comfortable environment by being aware of risk management issues and concerns that could compromise safety or incur liability. Assists in program evaluations through interviews, observations, surveys to assess the effectiveness of restorative care program and future planning. Supports the Restorative Care Coordinator in maintaining and ensuring accessibility of equipment and inventory. Assists in the preparation of accurate, timely and current reports, documentation of services provided, and other data as required. Ensures the maintenance of accurate and up-to-date documentation. Strong communication skills with resident families, staff, colleagues, and relevant internal and external partners. Reports incidents or complaints in a timely manner to the Associate Director of Care and in compliance with applicable policy and legislation. Responds to inquiries and resolves concerns, as appropriate. Clean and disinfect the restorative care equipment following the IPAC best practice guidelines. Ensures that services provided meet Regional customer service standards. Participates on committees, work groups, task forces and special projects, as assigned. Manages information in accordance with legislation and corporate standards. Performs other duties as assigned, in accordance with Branch and Department objectives.


WHAT WE'RE LOOKING FOR




Successful completion of a 2 year Community College Diploma in a Physiotherapy Assistant / Occupational Therapy Assistant /or Rehabilitation Assistant or related field or approved equivalent combination of education and experience. Minimum two (2) years experience in restorative care. As a condition of employment, candidates will be required to provide a satisfactory Police Vulnerable Sector Check (PVSC), tuberculosis screening and proof of immunization in accordance with the FLTC Act (O.Reg. 246/22). Knowledge of the Fixing Long Term Care Act, 2021, Occupational Health and Safety Act and any other pertinent Federal, Provincial and Municipal legislation that influence the operation of a nursing home. Ability to work with diverse residents, families, staff, and colleagues and comply with the Region's EDI policy and complete the relevant EDI educational requirements. Demonstrated ability in the Region's core competencies. Basic skills in virtual platforms and MS Office Suite. Good communication skills with resident families, staff, colleagues, and relevant internal and external partners. Demonstrated ability to work independently and as an effective team member in a cooperative manner with all members of staff, families, residents, volunteers, and internal and external partners. Ability to travel to offsite locations, as required. Ability to work outside regular business hours, as required.
