Job Description

Key Responsibilities


Provide individual, family, and group counselling using appropriate, client-centered social work methods with a focus on adoption and permanency. Conduct home study assessments, supervise adoption placements, and prepare necessary court and Ministry documentation to support the finalization of adoptions. Offer pre- and post-adoption counselling, including assistance with adoption disclosure, background information, and reunification with birth relatives. Maintain sealed adoption files, including compiling non-identifying medical and social histories. Stay current on adoption-related legislation, regulations, and procedures, and ensure services comply with Ministry standards and agency policies. Collaborate with internal teams and external partners (e.g., health, education, legal, and community agencies) to support case plans and client needs. Advocate for client access to adoption records, services, and supports within and outside the agency. Provide support to foster, kinship, and adoptive families, including conducting SAFE assessments and contributing to family education initiatives. Assist in developing and implementing policies, procedures, and group services related to permanency planning. Support administrative coordination of complex cases and contribute to cross-organizational projects and staff mentoring. Maintain accurate and timely documentation in compliance with PHIPA, CYFSA, and agency protocols. Represent the agency's mission, vision, and values in professional settings, and participate in evaluation and continuous improvement efforts. Perform other duties as required to support the Resource and Children in Care teams.

What You Will Bring:


Bachelors in Social Work or equivalent degree. 3 years' experience in adoption or child welfare related position. Registered Social Worker/Member of OCCSW in good standing is an asset. Extensive knowledge of the Child & Family and Youth Services Act in Ontario, the field of adoption and the relevant child welfare legislation in Ontario concerning this area. Access to a vehicle is required. Excellent communication & interpersonal skills.
