Overview

We are seeking a dedicated and organized Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that patients receive the highest level of care by facilitating communication between patients, healthcare providers, and administrative staff. This position requires a strong understanding of medical terminology, as well as experience in medical or dental office settings. The Care Coordinator will be responsible for managing patient information and coordinating care plans to enhance patient outcomes.

Duties

Serve as the primary point of contact for patients, addressing their inquiries and concerns regarding their care. Coordinate appointments, referrals, and follow-up care to ensure continuity of services. Maintain accurate and up-to-date electronic medical records (EMR) for all patients. Collaborate with healthcare providers to develop and implement individualized care plans. Educate patients about their treatment options, medications, and follow-up procedures. Ensure compliance with all regulatory requirements and maintain confidentiality of patient information. Assist with administrative tasks such as scheduling, billing inquiries, and insurance verification as needed.

Qualifications

Proficiency in medical terminology is essential for effective communication within the healthcare environment. Previous experience in a medical office or dental office setting is preferred. Familiarity with electronic medical records systems is highly desirable. Strong organizational skills with the ability to manage multiple tasks simultaneously. Excellent interpersonal and communication skills to interact effectively with patients and staff. A background in office administration or support roles is beneficial.
