Temporary Full-Time (6 month contract - possibility of extension)
Wage range: $37.55 - $45.36
The primary goal of the Social Worker is to ensure our patients are receiving high quality, person-centered care that aligns with our values of Dignity, Service, Excellence and Integrity. Our Social Workers work closely with patients and their families to restore, maintain and enhance the patients' quality of life by optimizing strengths, coping capacities and resiliency. They support patients as they relate to others, address environmental stressors and provide psychosocial education related to wellness and well-being as an integral component of their slow-paced rehabilitation programs. The Social Workers collaborate as part of the interprofessional team to address the social and emotional impact of physical and cognitive impairment, with an emphasis on identifying solutions to issues that interfere with optimal quality of life.
Additional duties include:
Providing support before, during and after transitions of care
Maximizing the coping capacities of patients and families and facilitating crisis intervention as needed
Performing psychosocial assessments and evaluating patients' self-assessments of quality of life
Participating in discharge planning and linking patients with community services, resources and opportunities
QUALIFICATIONS:
Bachelor's degree in Social Work from an accredited university
Active registration with the Ontario College of Social Workers
At least two years of experience in health care. Experience in geriatrics and/or rehabilitation preferred.
Bilingualism is considered an asset
