Position Summary

The role of the Virtual Care - Registered Practical Nurse in Virtual Care Services is to enhance patient and clinical practice outcomes through the facilitation and promotion of the various aspects of integrated virtual care services. This position aids in the development of Virtual Care practices that align with current best practices and alignment with services and programs offered by OSMH, the Couchiching Ontario Health Team (COHT) and other clinical partners as required.

PRIMARY RESPONSIBILITIES:

  • Identify opportunities where virtual care services can safely be offered and facilitated to enhance clinical care for patients and providers.
  • Meeting the College of Nurses of Ontario Standards of Nursing Practice for Registered Practical Nurses and functions within the policies and procedures of Virtual Care Services.
  • Organizing and implementing a plan of nursing care to meet the needs of each assigned patient.
  • Performing patient care activities and providing effective resources and support as appropriate.
  • Effectively utilizing human, physical and virtual resources.
  • Participate in initiatives to expand current virtual health offerings within the organization and with partner agencies.
  • Participating in the orientation of new staff members to the department as well as orientation of virtual care services and opportunities to stakeholders at large
  • Attending educational programs and in-services to enhance their knowledge, and participating in the planning and implementing of same.
  • Maintaining professional competence by assessing their own professional development needs, and establishing goals and actions to meet same.
  • Assisting in maintaining the established standards of care through individual performance, participation in hospital committees, and assisting with performance improvement, work and patient flow activities.
  • Maintaining an awareness of current developments in nursing practice.
  • Responsible to contribute to ongoing patient safety.
  • Functioning in accordance with established hospital emergency procedures.
  • Promoting collaboration by effective communication.
  • Participating as a member of the interprofessional team and promoting effective liaisons between patient, family, and physician, and other health team members and community agencies. Encouraging referrals to appropriate community agencies.
  • Demonstrating knowledge of and maintaining competence in accordance with RHPA and other relevant legislation and standards.
  • Participating in assessing health needs and promoting health education and maintenance through patient and family teaching programs.
  • Flexibility in adjusting patient assignments to facilitate urgent/emergency/acute treatments.
  • Performs other duties as may be assigned by the direct supervisor.
Qualifications

Education:
  • Current registrations with the College of Nurses of Ontario (CNO) as Registered Practical Nurse
  • Qualified to practice within current CNO RPN Scope of Practice Guidelines
Experience:
  • Minimum of five (5) years\' experience in the acute care setting required.
  • Proficient knowledge of: Microsoft Word, Outlook, Excel, Teams, OTN, ZOOM and various Internet browsers required.
  • Experience utilizing various electronic or audio-visual tools in the workplace is required.
  • Experience with Virtual Care preferred.

  • Demonstrated critical thinking skills as well as experience in initiating or managing change.
  • Demonstrated skill in patient education and promotion of patient independence using principles of adult learning.
  • Demonstrated appreciation and respect for protocol, professionalism, and confidentiality.
  • Demonstrated accountability for decision-making.
