Davenport-Perth Neighbourhood and Community Health Centre (DPNCHC)

is a multi-service community agency providing programs and services to diverse participants who are marginalized due to socioeconomic barriers. We work from a community development and health promotion perspective. Our Health Centre operates a busy, complex practice, and is a multi-disciplinary team of physicians, nurse practitioners, nurses, dietitian, and physiotherapist. We are seeking a Registered Practical Nurse to join our clinical team. We have expanded our services to deepen our work with Black-identified community members and youth. We recognize the importance of ally ship and the critical role allies play in affecting change. The successful candidate will be expected to work one evening a week, some weekends and work independently and as a part of a team.

Responsibilities:



As part of our clinical team, provide ongoing primary health care to clients within the scope of a registered practical nurse, including history taking, physical assessment, health teaching and health promotion, screening, referral education, advocacy and follow-up; Practice within the standards, medical directives, and scope of practice maintained by the Centre, and the College of Nurses of Ontario; Ensure appropriate, comprehensive treatment and continuity of care through maintaining complete and accurate client records; Participate in primary health care program and service delivery within the broader community context in partnership with other DPNCHC services and in various community settings, such as community programs, shelters and health fairs; Help to coordinate and support health centre intake processes and conduct intakes with new patients of the health centre; Incorporate best practices into care based on changing demographics and evolving needs of the community; Participate in program development, delivery and evaluation and engage in committee work within the broader community context in partnership with other institutions and agencies as required.

Qualifications:



Diploma in Practical Nursing from an accredited institution; Current registration with the Ontario College of Nurses in good standing; Excellent oral and written communication skills in English; Second language considered an asset; 6 months experience post registration in a community setting; Previous experience in community based primary health care within a culturally diverse community, with corresponding knowledge and commitment; Strong interest in working in the community with people who are precariously housed, living with mental health and or substance use issues and who have complex health needs; Strong commitment to health promotion and community development; Strong commitment to practice within an inter-disciplinary environment; Effective counselling and group development skills; Knowledge of pharmacology and the health care system; Ability to work collaboratively within and across teams; Strong time management skills as well as ability to prioritize tasks; Understanding of anti-oppression principles and ability to interact positively with people of diverse cultures and marginalized populations; Interest in exploring innovative service delivery approaches Knowledge of the Davenport Perth neighbourhood; Computer literacy.

Salary Range:

$53,103- $56,649 per annum, plus extended Health Benefits and Healthcare of Ontario Pension Plan (HOOPP).
