Job Description

The Complex Care and Recovery Program is currently seeking a full-time, temporary (12 month) Registered Practical Nurse to work with the Downtown Outpatient Clinic's Assertive Community Treatment Team (ACTT). The team follows an inter-professional field-based shared care case management model of recovery-oriented mental health care and provides intensive psychiatric treatment; medication administration and monitoring; and rehabilitation and support services for individuals with serious mental illness and complex needs including physical comorbidities, substance use and behavioral issues who find it difficult to engage in other mental health services. In addition, the team helps clients to access community services and assists them with activities of daily living. The goal is to support these individuals in their desire to live in the community by providing responsive, assertive and frequent contact, and ultimately to transition them to a less intensive level of care in the community.

Reporting to the Manager, you will coordinate care as the primary worker for a number of clients, while being available to the team's other registered clients and contributing in a discipline specific role. You will provide client-centered psychiatric and general nursing care. In addition, you will practice in accordance with the CNO standards, as well as CAMH Policies and Practice Model. You will perform assessments, develop, implement and evaluate the interprofessional plan of care for clients. You will communicate with clients' social and community networks, and assess new referrals to the program. You will liaise with other services in the Complex Mental Illness Program and community agencies to coordinate care for clients. You will work in partnership with clients, their formal and informal supports, and the team, to support clients' successful integration into their communities. You will be able to work with clients from diverse populations; contribute to program development; participate in clinical supervision; provide education support and supervision to students and participate in the research activities of the program. You will utilize your sound knowledge of psycho-social rehabilitation, crisis intervention, addiction issues, recovery-oriented care, groups' facilitation, as well as severe and persistent mental illness. You will support and contribute to a healthy workplace environment that embraces diversity, encourages teamwork and complies with all applicable regulatory and legislative requirements. Weekend and evening shifts will be required. This position is located at 393 King St E.

Qualifications
You must be a registered member in good standing as a Registered Practical Nurse with the College of Nurses of Ontario and possess a current B.C.L.S. (CPR) certification. You must have medication administration and health assessment certification, or have completed a Registered Practical Nursing Diploma program that included medication administration and health assessment. You will have two years of nursing experience. Preferred experience with individuals with mental illness and/or addictions, and/or physical comorbidities. Experience within an outpatient setting is an asset. In addition to your specific experience in psychiatric assessments, you will possess effective decision- making, problem solving, proven organizational skills, and the ability to facilitate work within a collaborative, interprofessional environment. You will possess excellent communication and interpersonal skills to enable you develop the Nurse-Client relationship and work within a collaborative and diverse inter-professional team. Preference will be given to candidates who possess a strong knowledge of psychiatric nursing and addiction issues and the Mental Health Act. Experience working with individuals from diverse ethno cultural and socioeconomic backgrounds is required. Computer literacy is also required. Bilingualism (French/English) and/or proficiency in a second language would be asset.

Salary Range: $28.18 - #33.65 per hour
