POSITION: Registered Practical Nurse
UNIT: Continuing Care Centre (CCC)
STATUS: Regular Part Time
SALARY: $37.82 - $38.20

WHAT YOU MUST HAVE:

• Graduate of a Registered Practical Nurse program from an accredited institution
• Registered Practical Nurse, currently registered and in good standing with the College of Nurses of Ontario
• Current BLS Certification
• Demonstrated problem solving, critical thinking, decision making, and conflict resolution skills and able to work through conflicts and differences in a respectful manner
• Recent experience in physical assessments, IV medication administration, administration of blood and blood products, catheterization, chest and respiratory assessment with the ability to demonstrate competencies
• Acceptable performance record
• Excellent attendance record

WHAT WE PREFER YOU TO HAVE:

• A minimum of one (1) year of Continuing Care experience within the last three (3) years
• Continuing education in geriatric nursing
• Certificates in: Palliative Care Fundamentals (Level 1), Gentle Persuasive Approaches, Crisis Intervention

WHAT YOU WILL BE DOING:
The RPN works at their full scope of practice to provide direct care to assigned patients. The RPN practices independently when the patients condition is less complex such that the outcome is predictable. The RPN collaborates with the Registered Nurse in patient situations that are beyond the RPN scope of practice.
Three levels of care are provided in our CCC unit that include:

• Activation / Restoration with a functional goal of progression (i.e. exercise and recreational activities) to increase and strengthen independence. The average length of stay in this program is 56 to 72 days.
• Short Term Complex Medical Management with a functional goal of stabilization and progressions (i.e. medically complex and specialized services) to help avoid further loss of function, increase activity tolerance, and progress. The average length of stay in this program is up to 90 days.
• Long-term Complex Medical Management with a functional goal of maintenance (i.e. medically complex and specialized services over an extended period of time) to maintain/slow the rate of, or avoid, further loss of function.

HOURS:
The successful candidate must be available to work all tours applicable to the department in accordance with the availability and non-availability requirements outlined in the Collective Agreement. In addition, this position must be flexible to occasional changes in hours when required to meet the operational needs of the department.
