The Jean Tweed Centre is building capacity to transform the level of support for women with substance use, mental health, and gambling concerns.

The Integrated Care Initiative at Broadview offers withdrawal management support, an intensive substance use program and transitional housing geared to the specific needs of women with substance use concerns. All services are co-located in one location in Toronto to support easy transitions along a continuum of care.

The program at Broadview comprises of a live-in treatment program inclusive of a 4-bed withdrawal supportive stay component for women who are transitioning to the intensive substance use live-in treatment as well as an aftercare supportive stay component. The withdrawal management supportive stay beds will provide access to women-specific withdrawal supports in a community-based approach for clients as they transition to live in treatment program beds, or other services within Jean Tweed Centre (or elsewhere in the system as desired). The withdrawal supportive stay program will provide non-acute withdrawal management support for clients who can be safely supported in a community support setting who do not require intensive medical withdrawal management intervention. This role reports to the Manager, Outpatient Services.

How will you help to get us there?



As a Registered Practical Nurse at the Withdrawal Management Support Services you will:

Assess, identify, plan, intervene and evaluate bio-psycho/social/spiritual care needs in accordance with best practices, and document same electronically and/or manually. Develop and implement client care plans based on assessment and in conjunction with the client; explains care plans to clients; and modify care plans as required. Identify client education needs and arrange learning activities related to substance misuse, mental health challenges, medications and health care. Conduct individual substance use/dependence histories/assessments and psychiatric and mental health assessments; collect and document client status; report status and changes in status to the inter-disciplinary team. Respond to medical emergencies, and rapidly changing situations; and initiate measures to relieve crisis or emergent situations. Manage clients' medications through the complete cycle of obtaining/liaising with the community pharmacy, verifying medication, dispensing medication, and ensuring onsite quality and safety of the medication reconciliation process, handling and monitoring for the program. Provide nursing interventions and treatments as ordered by Nurse Practitioners and/or physicians Complete all client file documentation in accordance with JTC policies and procedures and in accordance with the documentation standards of the Colleges of Nurses of Ontario (CNO) Collaborate and participate as a member of an inter-disciplinary team in formal and informal client case conferences. Conduct and facilitate individual and group interventions with clients; utilize therapeutic elements in a group process and individually. Contribute to the ongoing development of the service; provide input into the development and/or revision of policies and procedures and provide input to and participate in quality improvement plans/activities. Performs other related duties as assigned.

What will you bring?



Registered Practical Nurse with current registration and good standing with the College of Nurses of Ontario 2-3 years nursing experience working with those impacted by substance use and mental health challenges in either an inpatient unit or community setting. Demonstrated passion for change and ongoing improvement in support of patient care and safety Demonstrated knowledge, understanding and application of recovery, physical health assessment, harm reduction, substance use treatment, management of intoxication and withdrawal, motivational interviewing and trauma informed approaches. Demonstrated knowledge and familiarity with Opioid Agonist Therapy and withdrawal management interventions. Demonstrated knowledge and application of legislation which guides mental health and addiction nursing practice (example, Mental Health Act, Health Care Consent Act). Ability to communicate effectively, both verbally and in writing Proficiency in computer skills Ability to exercise critical thinking skills to analyze and resolve problems. Demonstrated ability to work independently and as part of an inter-disciplinary team.
