Job Description

Job Summary:

We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our healthcare team for a long-term position. The RPN will provide high-quality nursing care to patients, collaborate with healthcare professionals, and ensure the comfort and well-being of patients in a variety of healthcare settings, including hospitals, long-term care facilities, and home care environments. The role requires a commitment to patient-centered care and the ability to work independently and as part of a team.

Key Responsibilities:

Patient Care

: Provide direct patient care, including assessments, monitoring vital signs, administering medications, and performing nursing procedures as per physician orders and care plans.

Health Monitoring

: Observe and monitor patients' physical, emotional, and psychological status and report changes to the nursing supervisor or physician.

Treatment Administration

: Administer prescribed treatments, medications, and therapies; ensure accurate documentation of procedures and patient responses.

Collaboration

: Work closely with Registered Nurses (RNs), physicians, and other healthcare professionals to develop, implement, and adjust care plans.

Patient Education

: Educate patients and families on health management, wellness, and post-discharge care, including medication administration and lifestyle changes.

Documentation

: Maintain accurate and up-to-date patient records, including progress notes, treatment plans, and other documentation as required by law and facility standards.

Emergency Care

: Respond promptly to patient emergencies, provide first aid, and assist in emergency procedures as needed.

Infection Control

: Adhere to infection prevention protocols, maintain cleanliness, and ensure patient safety in accordance with health standards and regulations.

Patient Advocacy

: Act as a liaison between patients, their families, and the healthcare team, ensuring the best possible care and patient experience.

Compliance

: Ensure compliance with all relevant healthcare regulations, standards, and ethical practices.
Qualifications:

Education

: Graduate of an accredited Practical Nursing Program.

Licensing

: Valid and current registration with the [appropriate provincial or national nursing regulatory body, e.g., College of Nurses of Ontario (CNO)].

Experience

: Minimum of 2 years of nursing experience in a long-term care, hospital, or similar healthcare setting. Experience in geriatric care is an asset.

Skills

: Strong clinical and critical thinking skills. Ability to work independently and as part of a multidisciplinary team. Excellent communication and interpersonal skills. Compassionate and patient-centered approach to care. Strong attention to detail and organizational skills. Ability to handle stressful situations with professionalism.
Working Conditions:

Full-time, permanent position with 30 hours per week. Shifts may include evenings, weekends, and holidays. Physical demands include standing, walking, and lifting patients or equipment. Opportunity for career development and training.
