Job Summary

We are seeking a compassionate and skilled Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will have a strong background in patient care across various settings, including nursing homes, hospitals, and home care. This role requires excellent clinical skills, the ability to work collaboratively with healthcare professionals, and a commitment to providing high-quality care to patients of all ages.

Duties

Provide direct patient care, including monitoring vital signs and administering medications. Assist in the development and implementation of patient care plans. Perform phlebotomy and other laboratory procedures as needed. Document patient information accurately using electronic health records systems such as Epic or Cerner. Collaborate with interdisciplinary teams to ensure comprehensive patient management. Offer support and education to patients and their families regarding health conditions and treatment plans. Maintain a safe and clean environment for patients, adhering to infection control protocols. Provide specialized care for patients with dementia, pediatrics, senior care needs, or those requiring hospice services. Participate in case management activities to coordinate patient services effectively.

Requirements

Valid Registered Practical Nurse license in the applicable state or province. Experience in nursing home settings, hospitals, or acute care facilities is preferred. Knowledge of medical terminology and medication administration protocols. Familiarity with electronic health record systems (Epic, Cerner) is an asset. Strong communication skills and the ability to work well within a team environment. Previous experience in home care, assisted living, or hospice care is advantageous. Understanding of pain management techniques and laboratory processes is beneficial. Compassionate approach towards patient care with an emphasis on dignity and respect. Ability to adapt to various patient needs including pediatrics or senior care.
